Acclaim Multispecialty Group, a partner with JPS Health Network, is seeking a motivated advanced practice provider to join our General Family Medicine team. The role involves treating complex medical issues for patients in an outpatient setting within a county hospital system and urban teaching hospital environment. The hospital is located in Fort Worth, in the Magnolia Village and Medical District area. Job Summary
Outpatient Clinic Patient panel – Primary Care – Adult and Pediatric Responsibilities
Coordinate the multidisciplinary care delivered to their panel of patients to optimize patient care. Review patient medical records to diagnose, treat, and provide care management and education to patients and families. Make appropriate patient referrals. Monitor national health guidelines and those established by JPS Health Network and Acclaim Multispecialty Group for the management of patient care. Provide evidence-based and compassionate care to all patients to improve outcomes and increase patient satisfaction scores. Demonstrate patient-centered and patient-valued behavior. Maintain a collaborative environment with referring physicians, residents, medical students, advanced practice professionals, staff, and support staff to provide quality patient care. Support academic, educational and research endeavors. Attend Performance Improvement Programs and Peer Review committee meetings; actively participate in the process. Partner and communicate to build a positive brand and patient/customer experience for those who encounter JPS Health Network and Acclaim Multispecialty Group, including staff, vendors, outside partners, patients, stakeholders, and community leaders. Serve as a resource to the community. Perform other job-related duties as assigned. Knowledge, Skills and Abilities
Comfortable working in a large public/county facility. Working knowledge in the assigned area with competency in assessment techniques, diagnosis and treatment processes applicable to the medical specialty. Good interpersonal, verbal and written communication skills to work effectively with health care professionals and serve as a community resource. Self-directed with initiative to develop and participate in community activities and medical outreach programs related to the medical specialty area. Qualifications
Required Education and Experience: Graduate of an accredited Nurse Practitioner program with an active Texas License; must have current Texas APRN license with ACNP certification OR Graduate of an accredited Physician Assistant Program with an active Texas License. Required Licensure/Certification/Specialized Training: Current, unrestricted medical license to practice in the State of Texas; Unrestricted work authorization to work in the United States. Benefits
