The Integrated Care Center Clinic is a federally qualified health center offering primary care, behavioral health, addiction treatment, psychiatry, dental care, and more. We serve individuals who are homeless or facing mental illness, substance use disorders, and chronic medical conditions. We provide team-based care in an integrated setting. The Enhanced Case Management program under the Integrated Care Center provides intensive, client-centered case management to support clients in achieving stability, health, and self-sufficiency. The Case Manager at ECM will maintain a caseload of members served under the Managed Care Plan (MCP), linking clients to appropriate services such as health benefits, mental health, substance use disorder, physical health, employment, housing, and other community resources. This role engages with ECM in partnership with San Francisco Health Plan and the Integrated Care Clinic, and coordinates with providers to meet the clinic’s patient population needs. The Case Manager identifies and engages with each member in the community, including the member’s home and other outreach locations.

KEY RESPONSIBILITIES Client Care and Care Coordination Responsibilities:

Organize patient care activities, share information with the multi-disciplinary care team, and implement activities identified in the Managed Care Plan.

Identify eligible people from State Prisons, County Jails, hospitals, and other locations for MCP enrollment.

Engage through multiple strategies (in-person, mail, email, texts, telephone, community outreach) and direct contact with members or advocates.

Maintain regular contact with all providers in the member’s care team, including Justice Involved oversight.

Ensure continuous and integrated care among all service providers; refer to and follow up with primary care, physical and developmental health, mental health, SUD treatment, and housing or other social services as needed.

Support treatment engagement by coordinating medication review/reconciliation, scheduling, reminders, transportation, accompaniment, and addressing barriers to engagement.

Engage with a multi-disciplinary care team to identify gaps and coordinate input for primary, behavioral, developmental, oral health, and Community Supports.

Assist with intake by completing assessments as required by the MCP.

Monitor progress toward Care Management treatment plan goals and provide input on the treatment plan.

Assess the client’s Care Manager needs and complete all Releases of Information.

Connect clients to benefits, healthcare services, employment, housing, and other resources.

Coordinate communication and external service linkage, including scheduling, probation contacts, child visits, and court-related communications; provide appointment reminders for therapy and psychiatry.

Collaborate with clients and families to support reintegration into the community and work with outside agencies to obtain needed resources.

Clinical Documentation and Administrative Responsibilities:

Write and complete all progress notes within 24 hours of service delivery.

Prepare progress letters and court reports.

Complete assigned peer reviews (chart audits) promptly.

Actively participate in group supervision, agency, and team meetings.

Participate in training opportunities and complete assigned training in a timely manner.

Training, Supervision and Quality Improvement Responsibilities:

Attend and participate in case conferences to advocate collaborative discussion for treatment needs.

Participate in agency and team meetings and complete training requirements in a timely manner.

Collaborate with all members of the behavioral health team and complete Relias course assignments.

Field-Based Responsibilities:

This is a field-based position requiring frequent travel. The Lead Care Manager will meet ECM members where they are, including at their homes or other locations, and will have access to an office for case management activities, supervision, and training as needed.

QUALIFICATIONS Education, Certification, or Licensure

Case Manager I:

High school diploma or equivalent required.

Case Manager II:

Bachelor’s degree in a human services-related field (social work, public health, psychology) preferred.

Minimum two years’ experience in the human services field and/or expertise in substance use treatment, relapse prevention, and recovery preferred.

Current First Aid and CPR certification.

Valid California driver's license and access to insured transportation.

Experience with clients experiencing acute withdrawal from substances; trauma-informed services preferred.

Background Check and Other Requirements

Qualified candidates with arrest and conviction records will be considered for employment.

Must be able to complete a background check and livescan.
