$110,000.00/yr - $125,000.00/yr What you’ll do as a Home Health Speech Therapist/Language Pathologist (ST)
In partnership with the Case Manager you will provide skilled services to assist patients with breathing, speaking, and swallowing functions. You will evaluate and treat speech, language, cognitive and fluency disorders that affect communication. You will serve as an educator, patient advocate and a member of the interdisciplinary care team. Responsibilities
Provide skilled services in collaboration with the Case Manager to support patient communication, swallowing and respiratory needs. Assess and treat speech, language, cognitive, and fluency disorders. Act as an educator and advocate within an interdisciplinary care team. Qualifications
Current unrestricted CA Speech Therapist License Current BLS/CPR Certification Valid CA Driver's License Knowledge of individual development and human behavior as it relates to illness/injury and the influence of culture on health care Knowledge of state and federal homecare regulatory guidelines (preferred) Experience in home health care (highly preferred) Experience with high acuity patients is a plus Experience working with an interdisciplinary team (highly desired) Case Management fundamentals (highly desired) Self-Directed, empathetic, critical thinker with outstanding interpersonal skills and leadership abilities Organized and able to manage competing priorities Pre-employment computer literacy/competency assessment required prior to scheduling an in-person interview Why you should come work for us as a Speech Therapist/Language Pathologist (ST)

Anticipated wage range for Speech Therapist: 110,000.00 - 125,000.00. Starting pay is commensurate with relevant experience above the minimum requirements. 21st Century offers generous bonus incentive plans, comprehensive health benefits and 401k (up to 4% match).
