Pay Base pay range: $110,000.00/yr – $125,000.00/yr.

Anticipated wage range: $110,000 – $125,000 per year.

Location Territory: Solano County – Vallejo, American Canyon, Hercules, with minimal visits to Napa County. Candidates must be currently located in or planning to relocate to this area.

About the Role As a Home Health Speech Therapist/Language Pathologist (ST), you will partner with Case Managers to provide skilled services that assist patients with breathing, speaking, and swallowing functions, evaluate and treat speech, language, cognitive, and fluency disorders, and serve as an educator, patient advocate, and member of the interdisciplinary care team.

Evaluate and treat speech, language, cognitive, and fluency disorders that affect communication.

Serve as an educator and patient advocate.

Collaborate with the interdisciplinary care team.

Qualifications

Current unrestricted California Speech Therapist license.

Current BLS/CPR certification.

Valid California driver’s license.

Knowledge of individual development and human behavior as it relates to illness/injury and cultural influences on health care.

Knowledge of state and federal home‑care regulatory guidelines (preferred).

Experience in home‑health care (highly preferred).

Experience with high‑acuity patients (a plus).

Experience working with an interdisciplinary team (highly desired).

Case management fundamentals (highly desired).

Self‑directed, empathetic, critical thinker, outstanding interpersonal skills, leadership skills, organized.

Computer literacy and competency assessment required before in‑person interview; results are confidential and used as one of multiple data points in hiring decisions.
