As the Social Work Care Manager for Hoag Memorial Hospital, Newport Beach, you’ll be responsible for collaborating with Hoag medical staff and the community they serve to ensure an environment of extraordinary patient care. You will coordinate appropriate post-acute services for the patient based on the clinical assessment by physicians, nurses, and ancillary staff. You will consider patients’ needs through the continuum of care and offer psychosocial support to patients and families for adjustment to illness, grief counseling, and psychiatric stabilization. Inside The Care Management Department: Our Care Management team provides streamlined coordination of patient care services to Hoag patients and families. From admission to post-discharge, our Care Management team ensures care is appropriate, timely, cost-effective, and of high quality. We look to our Care Managers to fully support and provide the Hoag Experience for our patients. We value excellent communication and teamwork, effective listening skills, and strong customer service expertise. Responsibilities

The Care Manager (MSW or LCSW) coordinates patients’ needs through the continuum of care from pre-admission through post-discharge plans. This role works in collaboration with physicians, nurses, clinical staff, and community agencies to identify and arrange for appropriate care. Responsibilities include: Review clinician assessments and patients’ financial, family, and psychosocial support to develop comprehensive care and/or discharge plans. May focus on discharge planning, utilization review, and/or psychosocial support. Review records to assess admission status, level of care, payer source, and UR contracts to validate billing. Provide psychodynamic intervention and crisis counseling to support patients and families. Educate patients and families on healthcare options and connect them with resources. Document pertinent patient issues, contacts, and plans in the medical record. Serve as a mandated reporter for elder, child, and spousal abuse. Community Care specifically provides Care Management support to high-risk, homeless, and mental health populations. Perform other duties as assigned. Provide care in the outpatient environment and in the surrounding community as dictated by Community Benefit parameters. Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered. Manage client needs through the continuum of care including intake through case termination plans. Assess patient needs including financial, family, and psychosocial support, and develop tailored care plans. Record client progress including referrals, home visits, and other notable interactions; periodically evaluate progress and adjust to improve outcomes. Collaborate with Center for Healthy Living staff, partner agencies, and other community agencies as needed. Provide care to higher-acuity patients and work with team members to arrange appropriate resources. May provide psychodynamic intervention and crisis counseling; facilitate psychoeducational groups as needed. Assist in training interns and contribute to data collection to measure program outcomes. Perform other duties as assigned. Qualifications

Education and Experience Fulfills mandatory stroke education requirements per certification agency. Positions Requirements Depend On Licensure And Are As Follows Care Manager (MSW) – Master’s degree in Social Work (MSW) required. One year of MSW experience in an acute healthcare setting preferred. Care Manager II (LCSW) – Master’s degree in Social Work (MSW) required. One year of supervisory experience, preferably within an acute healthcare system environment. Five years’ of clinical or Social Work experience in an acute healthcare system environment. Bilingual or multilingual preferred. Community Health

(MSW required). One year of MSW experience in community case management preferred. Understands working with low-income and under-resourced communities. Must be a licensed driver with an automobile that is insured and in good working order. Bilingual or multilingual preferred.
