Position: Part Time Physical Therapist Case Manager Coverage Area: Santa Cruz County* Responsibilities
Perform an in-home evaluation and develop a care plan; work with Physical Therapy Assistants to follow the plan. Help patients with mobility, movement, and regaining independence. Create detailed treatment and care plans for each patient. Communicate and coordinate with patients’ physicians to ensure high-quality care. Evaluate and report patients’ progress to the physician or medical director. What you’ll do as a Home Health Physical Therapist Case Manager
Provide skilled services to evaluate and address home-based needs so patients can live safely at home. Assess and treat patients using medically prescribed physical therapy programs. Supervise Physical Therapy Assistants per agency and regulatory standards. Why you should join us

Current unrestricted CA Physical Therapist License Current BLS/CPR Certification Valid CA Driver’s License Knowledge of local community resources Understanding of development, behavior, and cultural influences on health care Knowledge of state and federal homecare regulatory guidelines (preferred) Home health care experience (highly preferred) Experience with high-acuity patients (a plus) Experience working with an interdisciplinary team (highly desired) Case management fundamentals (highly desired) Self-directed, empathetic, critical thinker with outstanding interpersonal skills Leadership and organizational abilities Salary: $120,000 - $135,000 per year, plus incentives as detailed by the agency’s compensation plan. Starting pay is commensurate with experience; benefits include health, 401(k), and other programs. Promotions may occur as responsibilities expand.
