Primary Care Physician | PACE Program

Join a mission-driven,value-based care team as a Primary Care Physician in our PACE (Program ofAll-Inclusive Care for the Elderly) center located in Stockton, CA. We areseeking full time physicians who are passionate about delivering holistic,patient-centered care to older adults through an integrated care model thatsupports clinical autonomy and meaningful relationships with patients. As a Primary Care Physicianin our PACE program, you will provide comprehensive primary care to frailseniors, collaborating with an interdisciplinary team to ensure optimal healthoutcomes. This role includes clinical leadership and offers a uniqueopportunity to make a lasting impact in a supportive, team-based environment.
