Primary Care Physician (Pod Leader)
with a nationally recognized PACE program, you'll have the time, support, and structure to truly focus on
healing, not volume.
Our model is different and so are you. You'll care for a small, stable panel of participants, lead an interdisciplinary care team, and deliver
whole-person, community-based care
that honors dignity, purpose, and connection. This is not traditional primary care; its medicine practiced as it was meant to be:
personal, proactive, and profoundly human.
What You’ll Do:
Provide direct, compassionate, and comprehensive primary care to a select panel of seniors within the PACE program.
Partner with nurses, social workers, therapists, and home health specialists to coordinate personalized care plans.
Perform in-person assessments, guide care transitions, and ensure long-term wellness goals are met.

8 patients per day , enabling true depth of care.

What You Bring:
Board-certified (or board-eligible) in
Internal Medicine, Family Medicine, Emergency Medicine, or Geriatrics .
Eligible for Medicare/Medi-Cal participation.
At least 12 years of experience practicing primary care, ideally in a
PACE, managed care, or value-based model.
A compassionate, collaborative approach to medicine.
Desire to lead a team and make care decisions that prioritize quality of life.
