Overview

Community Resource Specialist II position at Beth Israel Deaconess Medical Center, Boston, MA. This role involves collaborating with social work and medical staff to obtain community resources for patients and families, providing orientation and ongoing training related to community resources, and responding to referrals related to domestic, sexual, and community violence, including survivors of interpersonal violence. Responsibilities

Develops liaisons with community and governmental agencies to facilitate negotiation of complex systems. Maintain a caseload with multiple patient contacts per day (phone, paperwork, face-to-face). Serve as primary health care contact for psychosocially complex patients when needed. Provide timely documentation in patients’ medical records (paper and electronic). Identify and coordinate resources to meet patients’ and families’ needs independently or with clinicians. Develop multifaceted service plans using local and state resources to meet complex needs. Make independent recommendations for services to patients/families. Participate in medical center and community task forces and represent Community Resource staff in planning and administration. Advocate for patients in meetings with providers and in other agencies; assist with accessing entitlement programs and necessary services. Coordinate maintenance of current resource information for staff dissemination. Provide training for new/less experienced CRS staff and for social work staff, students, and medical professionals. Engage in outreach to meet evolving patient needs; participate in community coalitions and events as indicated. Disseminate information about resources to the Social Work Department and team as appropriate. Qualifications

Required Qualifications:

High School diploma or GED required. Bachelor’s degree in Social Work or Human Services preferred. Licensed Social Worker preferred. 2 years related work experience required if a Bachelor’s degree is obtained. 3+ years of related work experience required without a related Bachelor’s degree. Experience with computer systems including web-based applications and Microsoft Office (Outlook, Word, Excel, PowerPoint or Access). Preferred Qualifications:

Licensed Social Worker (LSW) is preferred. Competencies

Age-based Competencies: Competent to provide patient care to Young Adult, Middle Age, and Elderly groups as specified. Physical Nature of the Job:
