The Community Resource Specialist II works under the direction of the manager or supervising clinician, practicing at an advanced level. Provides leadership to the Community Resource team in administration, program development, training, and oversight. Collaborates with social work and/or medical staff to obtain community resources for Medical Center patients and families. Within the developed area of expertise, provides orientation and ongoing training related to community resources to staff, students, medical professionals, patients and families. The CRS’s primary responsibility is to respond to patient and staff referrals related to domestic, sexual, and community violence, as well as referrals from the Adelante Project for human trafficking survivors. They are available to the CVPR staff and patients during office hours for consultation, provide community and medical center resources to survivors of violence, and may accompany patients to medical appointments or other external services as schedule allows. The CRS will be an active member of the CVPR and contribute to Team Meetings and the CRS group. The CRS functions as the primary resource knowledge holder for survivors of interpersonal violence and disseminates information to the team or Social Work Department as appropriate. Responsibilities
Provide sensitive, culturally relevant responses to survivors referred to the CVPR. Identify the specific needs of each person and arrange appropriate referrals and services; encourage and assist patients in using available services (e.g., EOHLC, Department of Transitional Assistance, health, mental health, and legal services). Advocate for patients in meetings with providers within the medical center and other agencies. Assist in program development related to resource needs and systems response. Engage in outreach to meet evolving patient needs and participate in outreach events. Co-facilitate support groups for the CVPR (days or evenings) with other CVPR staff. Participate in community-based coalitions and roundtables as indicated. Qualifications
Required Qualifications
High School diploma or GED required; Bachelor's degree in Social Work or Human Services preferred. Licensed Social Worker (LSW) preferred. 2 years of related work experience required if Bachelor's degree obtained. 3 or more years of related work experience required without a related Bachelor's degree. Experience with computer systems, including web-based applications and Microsoft Office (Outlook, Word, Excel, PowerPoint or Access). Preferred Qualifications
Licensed Social Worker (LSW) is preferred. Competencies

Independence of Action: Follow precedents and procedures; set priorities within general guidelines; seek supervision when needed.
