Provides sensitive, culturally relevant responses to survivors referred to the CVPR. Identifies the particular needs of each person and ensures appropriate referrals and services are located; encourages and assists patients in utilizing available services (e.g., Executive Office of Housing and Livable Communities, Department of Transitional Assistance, health, mental health, and legal services). Advocates for patients in meetings with providers in the medical center and in other agencies. Assists in program development related to resource needs and systems response. Performs outreach to meet evolving needs of patients seeking help at the medical center. Participates in outreach events across the medical center, affiliated providers, and community groups. Co-facilitates support groups for the CVPR (days or evenings) with other CVPR staff. Participates in community-based coalitions and roundtables as indicated. Functions as the primary resource for information on resources for survivors of interpersonal violence and disseminates information to the team or Social Work Department as appropriate. Responds to patient and staff referrals related to domestic, sexual, and community violence, including referrals from the Adelante Project (human trafficking survivors). Provides accompaniment to medical appointments and other external services as schedule allows. Provides community and medical center resources to survivors of violence, and maintains current resource information for staff use. Documents issues and obstacles to obtaining services and advises supervisory/clinical staff accordingly. Essential Responsibilities

Develops optimum liaisons with community and governmental agencies to facilitate negotiation of complex systems. Maintains a caseload with multiple patient contacts per day (phone, paperwork, and face-to-face). Acts as primary health care contact for psychosocially complex patients when needed. Provides timely documentation in patients' medical records (paper and electronic). Determines resources to meet patients’ and families’ needs independently and/or with clinicians. Uses knowledge of local and state resources to develop multifaceted service plans for complex needs. Makes independent recommendations on services for patients/families. Participates in medical center and community task force groups regarding patient care and programs. Represents Community Resource staff in departmental planning and administration. Keeps staff informed of procedural requirements and changes by state/local agencies. Advocates for patients in written and verbal communications. Assists patients and families with tasks like accessing entitlement programs. Provides medical information as requested by external facilities or agencies in line with HIPAA regulations. Facilitates orientation and ongoing training for new/less experienced CRS staff and trains social work staff, students, and clinicians on resources. Coordinates maintenance of current resource information for dissemination among social work staff. Documents and informs supervisory/clinical staff of obstacles to obtaining services requiring social action. Required Qualifications

High School diploma or GED required. Bachelor’s degree in Social Work or Human Services preferred. Licensed Social Worker preferred. 2 years related work experience required if Bachelor’s degree obtained; 3+ years required without related Bachelor’s degree. Experience with computer systems, including web-based applications and Microsoft Office (Outlook, Word, Excel, PowerPoint, Access). Preferred Qualifications

Licensed Social Worker (LSW) preferred. Competencies

Decision Making, Problem Solving, Independence of Action, Written and Oral Communications, Knowledge, Team Work, Customer Service. Age-based competencies: ability to provide patient care to ages 16-30, 30-60, and 60+. Physical nature: light work; capable of lifting up to 20 pounds; some sedentary work with movement around the hospital campus.
