Job Summary We are seeking an MDS Coordinator to join our team. The MDS Coordinator will be responsible for coordinating the completion of the Minimum Data Set (MDS) assessments for all residents in accordance with state and federal regulations. This role involves working closely with healthcare providers to ensure accurate and timely completion of assessments.
Salary Range Salary Range: $100,484.80 ($48.31) and willing to train
Medical Insurance (including Drug, Vision, and Dental coverage)
401K (We offer a 4% matching contribution, with Traditional and Roth 401k options)
10 paid holidays per year
10 days of paid vacation, with increases at 4yrs and 7yrs of service
10 days of paid sick leave

Qualifications
Bachelor's degree in Nursing or a related field.
Active Registered Nurse (RN) license.
Previous experience in MDS coordination or a similar role in a healthcare setting, helpful but not necessary.
Knowledge of MDS assessment process and regulations, helpful but not necessary.
Strong attention to detail and organizational skills.
Excellent communication and interpersonal abilities.
Responsibilities
Coordinate and complete MDS assessments accurately and timely.
Collaborate with healthcare providers to gather information for assessments.
Ensure compliance with state and federal regulations related to MDS assessments.
Review and update resident care plans based on MDS assessments.
Communicate assessment findings to appropriate healthcare team members.
Participate in quality improvement initiatives related to MDS assessments.
Maintain accurate and up-to-date records of assessments and care plans.
