JOB TITLE: Staff Nurse II
CC/Department: 7010/Emergency Services
Reports to: Administrative Director of Emergency Services
The Staff Nurse is a professional caregiver who assumes responsibility and accountability for the assessment, nursing diagnosis, planning, implementation and evaluation of care for patients and their families in compliance with the standards for care at Seton Medical Center, AHMC and Nurse Practice Act.
Responsibilities
Assesses, collects and documents health assessment data focusing on the physical, emotional, spiritual, psychosocial and educational needs of the patient.
Interviews the patient/family/significant other to complete triage information and identifies actual and potential needs and barriers to learning.
Completes and documents an assessment after initial triage, focusing on abnormal systems and actual/potential problems; performs ongoing assessment as indicated.
Plans nursing care based on priority of physical and psychosocial-emotional needs; develops realistic short-term goals involving patient, family or significant others.
Demonstrates understanding of age-related needs and negotiates with patient, family, significant other and health care team members for education and discharge planning.
Implements the plan of care including prescribed procedures and treatments; organizes and prioritizes care based on individual needs and unit activity; assesses changes in condition and appropriateness of the plan; initiates interventions using clinical expertise.
Acts as Primary Nurse, coordinating activities of health care team members in completing diagnostic and therapeutic interventions.
Evaluates the effectiveness of nursing care to reach expected outcomes; documents patient status and response to care concisely and accurately.
Utilizes professional nursing knowledge to assess for physical and psychosocial-emotional needs, barriers to learning, educational/discharge needs, age-related needs, and issues related to assault or abuse.
Analyzes patient data (vital signs, medical history, exam findings, labs) to determine appropriate care and uses advanced, specialized knowledge to implement interventions prioritized by patient needs.
Provides health education and discharge information that is age-specific and ensures patient/family understanding; initiates/monitors discharge planning as needed.
Performs nursing procedures within scope of practice (e.g., IV access, medication administration, dressings) in accordance with unit and hospital policies.
Performs other nursing procedures/skills (e.g., cardiac monitoring, rhythm interpretation, defibrillation, conscious sedation monitoring, ABGs) as trained and within scope and policy.
Recognizes and responds quickly to emergency medical situations; assesses A-B-C, initiates CPR and other emergency treatments as appropriate; communicates critical information to team members.
Maintains required credentials and training (CA RN license, BLS, ACLS, PALS, NIHSS, AB508); stays current on emergency treatments and best practices.
Attends staff meetings and continuing education; integrates new knowledge into clinical practice and demonstrates age-related care competency.
Adheres to state/federal regulations (Title XXII, COBRA/EMTALA) and follows TJC, CDPH, CMS guidelines; advocates for public health and safety through education.
Completes required documentation for Emergency Services visits consistently.
Demonstrates caring and compassionate conduct with patients, families, staff and physicians.
May perform duties of Relief Charge RN when called upon.
Adheres to departmental and medical center policies on scheduling, overtime, sick time and time off; punctuality and appropriate breaks and use of sick time.
Position-Specific Duties / Qualifications Education
Graduation from an accredited school of professional nursing.
Baccalaureate degree in Nursing preferred.
Experience
Staff Nurse II - Minimum of six months RN experience.
Six months current ER &_triage experience or completion of Seton ER Nurse Training Program.
Certifications/Licensure
Active California Registered Nurse licensure
Basic Life Support (BLS)
ACLS
PALS
NIHSS
Workplace Violence Prevention De-escalation 8-hr (AB508) or CPI
Preferred NRP certification
Other Skills, Abilities & Knowledge
Diffusing Assaultive Behavior Training
Knowledge of California Nursing Practice Act
Ability to follow all Nursing Service policies and procedures
Excellent communication and customer service skills
