Travel Medical Social Worker - $2,206 per week in Kent, WA

Job Title: Licensed Social Worker Profession: Social Services Specialty: LMSW (Licensed Masters of Social Worker) Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 2 years with geriatric population License: WA LICSW, WA LSWAIC, OR WA LASW required Certifications: BLS, MSW, and one of the social work designations listed above Must-Have: Previous experience working with geriatric population Description: This role involves providing social work services to participants. The services must align with the mission and core values of the organization. The position requires spending a majority of the time in direct service activities within the center. Tasks include one-on-one and group counseling, interaction with team members, and ongoing assistance to participants to address social issues. The candidate will conduct comprehensive assessments to provide perspective on social, emotional, cognitive, and ethnic factors. This input will help create individualized integrated care plans. The role also requires accessing community and financial resources for participants. Ensuring continuity of care across the continuum is essential. The candidate will conduct social work assessments and interventions in various settings, including participants' homes as needed. A moderate to light amount of fieldwork is involved, significantly less than in home health or hospice settings. Fieldwork will occur within a specified geographic area. Mileage and travel time between work locations will be reimbursed and considered paid time worked. All travel should occur within the workday during scheduled hours. Candidates will have autonomy in scheduling home visits but should not schedule them in connection with personal commuting. The position does not require weekend or on-call work. References and a minimum of supervisory experience are required. Floating between different facilities may be expected.
