Candidates who are interested in a permanent position after a 13-week contract are encouraged to apply. As a member of the interdisciplinary team, you will provide social work services to participants. You will spend a majority of your time in direct service activities, including one-on-one and group counseling. You will interact with team members and provide ongoing assistance to participants to address social issues. Based on comprehensive assessments, you will provide insights into the social, emotional, cognitive, and other factors that impact care plans. You will access community resources and ensure continuity of care across the service continuum. Conducting social work assessments and interventions in various care settings, including participants' homes, will be required. Moderate in-field work is expected, with some assessments done in participants' homes. The role includes autonomy in scheduling home visits. Travel during the workday will be reimbursed. Candidates will start from the center and will be required to perform home visits as part of their duties. You will be provided with a laptop and cell phone to carry out your responsibilities. A system will be used for tracking expenses and mileage. This role requires a master’s in social work and one of the specified state licenses. Two years of experience working with the geriatric population is required.
