Candidates interested in a permanent position after a 13-week contract are encouraged to apply. Open to travelers as well as locals. First-time travelers are accepted. This is a part-time position supporting an Adult Day Care Program. As a member of the interdisciplinary team, the candidate provides social work services to participants. The role is consistent with the mission and core values of the organization. The candidate will spend the majority of their time in direct service activities within the center. This includes one-on-one and group counseling. Interaction with team members and ongoing assistance to participants are key components of the role. The candidate will address social issues based on comprehensive assessments. They will provide perspective on social, emotional, cognitive, and ethnic factors impacting care. Individualized integrated plans of care will be developed for participants. The role involves accessing community and financial resources for participants. Continuity of care across the continuum will be ensured. Social work assessment and intervention in various care settings may be required. The candidate may conduct some assessments in the homes of participants. This involves moderate to light in-field work, significantly less than a home health or hospice social worker. Travel within a specified radius will be necessary for the role. Candidates will be reimbursed for mileage and travel time between work locations. The role offers autonomy in scheduling home visits. All visits will occur during the workday from 08:00 to 17:00. Mileage will be reimbursed when traveling between work locations, but personal commute will not be paid. Social workers will be assigned a laptop and cell phone to perform their duties. A system will be used to track expenses and mileage. The candidate is expected to float between facilities as needed. Travelers' refusal to float may result in termination of assignment. Candidates should have supervisory references covering recent employment within a comparable specialty. This position requires both a master's degree in social work and the necessary state licensure.
