Sunrise Senior Living is seeking a Wellness Nurse responsible for the overall well-being of residents, providing clinical support to the community team, and promoting high-quality resident care.
Responsibilities
- Identifies ongoing needs and services of residents to promote the highest quality resident services to be delivered.
- Ensures proper follow-through, assessment, and documentation for residents with changes in health care services and status.
- Assists Resident Care Director (RCD) in conducting resident assessments at move-in, 30 days post move-in, semi-annually, and with any change in status.
- Conducts monthly wellness visits for all residents.
- Assesses, monitors, and follows up with residents who have experienced any fall, accident, or incident; documents and reports as required.
- Documents all pertinent information in the resident wellness file and maintains communications with residents’ families to ensure medical needs are met.
- Contacts residents’ attending physicians when necessary or upon family request.
- Ensures weights and vital signs are obtained monthly for each resident and maintains wellness files according to Sunrise policies and regulations.
- Maintains medical supplies and emergency kits for the community and provides clinical support to the team as needed.
- Follows infection control practices and promotes infection control standards, including testing, training, and skills checks.
- Monitors each resident’s medication and treatment profile to ensure accuracy and adherence to orders.
- Demonstrates knowledge of quality improvement areas and regulatory requirements related to resident care and services.
Qualifications
- Graduate of an approved college/school of nursing.
- Current state RN license.
- Minimum of one (1) year of experience in home health, assisted living, hospital, or long-term care.
- Knowledge of nursing practices, techniques, and applicable regulations; strong assessment skills.
- Ability to handle multiple priorities; strong written and verbal communication skills; ability to teach and guide team members to provide quality clinical care.
- Competent in organizational and time management skills; good judgment, problem-solving, and decision-making abilities.
- Proficient in computer skills (Microsoft Office) and Sunrise applications; ability to learn new applications; ability to work weekends, evenings, and flexible hours as required.
Work Schedule
Part Time, 11:00pm-7:00am. Week 1 off; Week 2 Saturday and Sunday.
