Responsibilities
- Sunrise Team Member
- Identifies ongoing needs and services of residents to deliver the highest quality resident services.
- Ensures proper follow through, assessment and documentation for residents with changes in health care services or status.
- Assists Resident Care Director in conducting resident assessments at move-in, 30 days post move-in, semi-annually, and with any status change.
- Conducts monthly wellness visits for all residents.
- Assesses, monitors, and follows up with residents who have experienced any fall, accident, or incident; documents and reports as required.
- Documents all pertinent information in the resident wellness file; maintains communications with residents’ families to ensure medical needs are met.
- Contacts attending physicians as necessary and/or upon family request.
- Ensures weights and vital signs are obtained monthly for each resident.
- Maintains wellness files according to Sunrise policies and federal/state regulations.
- Maintains medical supplies and emergency kits for the community.
- Provides clinical support and assistance to the community team as needed.
- Understands and follows infection control practices; promotes infection control standards with the team and resident testing as required.
- Monitors residents’ medication and treatment profiles to ensure orders are followed and documented accurately.
- Demonstrates knowledge in quality improvement areas: Resident Centered Care Model, APIE, Quality Care Indicators and Outcomes; participates in Sunrise Quality Assurance and Clinical Meetings.
- Demonstrates knowledge of regulations: Federal, State, and Local resident care regulations; Resident Rights; Resident Assessment/Evaluation processes per regulatory requirements.
Qualifications
- Graduate of an approved college/school of nursing.
- Current state RN license.
- Minimum of one year experience in home health, assisted living, hospital, or long-term care.
- Knowledge of nursing practices, regulations, and good assessment skills.
- Ability to handle multiple priorities; strong written and verbal communication; ability to guide team members in clinical care.
- Strong organizational and time management skills; good judgment and problem-solving abilities.
- Proficiency in computer skills (Microsoft Office) and Sunrise applications; ability to learn new applications; ability to work weekends/evenings/flexible hours as needed.
