Overview HealthCare Access Maryland (HCAM) is a nonprofit organization dedicated to making Maryland healthier by connecting residents to health insurance coverage and providing comprehensive care coordination. HCAM assists over 80,000 individuals each year across Maryland, offering personalized guidance, health education, and advocacy to ensure access to optimal health.

Position Care Coordination Associate (Bilingual) – provides culturally competent services to the immigrant population in Baltimore City, including education and connection to health benefits, assistance with barriers to healthcare, and linkage to the Managed Care Organization, Primary Care Provider and/or Specialty care.

Base pay range $24.00/hr - $24.00/hr

Responsibilities

Educate clients on access to low-cost/no-cost health care services in Baltimore City if ineligible for MA programs.

Conduct home and telephone assessments to provide linkage and coordination to primary care, dental care, specialty care, preventive health screenings, lead screenings, immunizations and mental health services.

Educate clients about Maryland Medical Program benefits for eligible and potentially eligible clients.

Assist with removing barriers to healthcare including housing, addiction treatment, domestic violence services, Energy Assistance, emergency funds, and Medical Assistance Transportation.

Conduct home visits within the guidelines of Standard Operating Procedures.

Documentation

Carry an average caseload of 20 clients.

Close cases within 10 days for Pre-natal Risk Assessment (PRA) referrals and within 30 days for Local Health Service Request (LHSR), 1184, Post-Partum/Infant and Maternal referral (PIMR).

Document and close cases as defined within the Standard Operating Procedures.

Community Outreach

Provide outreach to the general and/or migrant population in Baltimore City, offering culturally competent services.

Collaborate with Baltimore City Public Schools to ensure eligible students have access to health care via the Maryland Children’s Health Program.

Human Resources

Timely and accurate submission of timesheets.

Attend and complete all required trainings.

Qualifications Education, Experience, Certifications and Licenses

High School Diploma or GED with significant human services experience. Bachelor’s degree in a Human Services field is preferred.

2 years of experience working in an administrative support capacity with a multidisciplinary team.

Ability to speak, read and write in Spanish.

Reliable transportation required.

Community Health Worker Certification (preferred).

Knowledge, Skills & Abilities

Strong computer literacy with MS Word, Excel, Outlook and SharePoint (365).

Organizational skills to meet a demanding workload.

Detail oriented to complete files and contract compliance.

Sensitivity to cultural and socioeconomic characteristics of the population served.

Housing First philosophy knowledge is preferred; support is required.

Excellent oral and written interpersonal and communication skills.

Problem solving skills; ability to work efficiently and meet deadlines.
