The MCH Program Manager supervises and supports MCH Initiatives/Centralized Intake Staff as part of the Care Coordination Program (CCP) at HealthCare Access Maryland (HCAM). The Program Manager will be responsible for representing HCAM in Baltimore Citywide initiatives to support the health and well-being of pregnant/postpartum women in the region. Duties and Responsibilities

Provide outreach strategies and coaching to the MCH Community Health Advocate/UTL team. Supervise Maternal and Child Health Initiatives staff – 6 Community Health Advocates (CHAs), conduct monthly audits, receive, track, review, and assign referrals for the CHA team. Work collaboratively with Referring Entities to ensure that pregnant women are connected and referred to HCAM for prenatal services and other community-based resources. Serve as a Liaison to the Baltimore City Public School systems “Judy Centers” to ensure successful staff outreach to these centers and timely referral of pregnant and postpartum women to care coordination from these locations. Participate in new hire orientation and onboarding curriculum highlighting the role of Centralized Intake System and B’More for Healthy Babies (BHB) Initiatives. Attend and participate in routine meetings as assigned but not limited to BCHD-BabyStat, HFA Home Visiting Meetings, BHB CAB, and MCH staff meetings. Be familiar with community resources and services in the city to support the MCH Initiatives team to connect pregnant and postpartum women and infants to improve pregnancy, birth, and infant health outcomes. Serve as the HCAM lead for group prenatal programming delivery and curriculum implementation. Case Management

Receive, track, review, and assign referrals for the CHA team. Conduct monthly audits and case reviews of cases assigned to the CHA team. Ensure 85% success rate for CHA outreach. Ensure 100% on-time closure for CHA outreach. Documentation

Complete monthly and quarterly required reporting for funders. Case reviews and monthly audits of CHA team. Performance evaluations and performance improvement plans. Human Resources

Approve staff time and PTO requests. Daily supervision of CHA team, including regular 1:1s and performance evaluations. Consistent and timely communication with supervisor-Deputy Director, MCHI. Complete and submit timesheets timely and accurately. Complete all required training courses. Lead recruitment efforts for all vacancies reporting to this role. Funder Relationships

Participation in B’more for Healthy Babies and Baltimore City Health Department work groups and strategic planning. Additional Duties and Responsibilities

Support of the Safe Sleep/MCHI Outreach Team as needed. QUALIFICATIONS

Bachelor's of Human Services or related field required, Master’s degree preferred. Experience in Leadership. Managing a team for 1-2 years required. At least four years of experience required providing home/community outreach, with priority given to those with experience working with the maternal and child health population in Baltimore City. Knowledge, Skills & Abilities

Demonstrated knowledge in the following areas: Community Outreach, Maternal and Child Health Disparities in Baltimore City, and Medicaid System Navigation. Demonstrated knowledge and experience working with HealthChoice Medicaid recipients and Managed Care Organizations. Strong computer literacy skills, specifically with MS Word, Excel, Outlook, PowerPoint, and SharePoint (365). Demonstrated proficiency in using Electronic Medical Records (EMR) systems to input, retrieve, and manage patient information accurately and securely. Organizational skills with the ability to meet a demanding workload. Sensitivity to cultural and socioeconomic characteristics of population served. Excellent oral and written interpersonal and communication skills required. Problem-solving skills. Proven ability to work efficiently and meet deadlines. Strong knowledge of community resources.
