Overview
Clinical Pharmacist - Outpatient Oncology and Infusion - PRN Under general supervision, facilitates the safe, effective and economic use of drugs in a fiscally responsible manner. Reviews prescriptions, obtains clarification as necessary and consults with medical staff, nursing personnel and others to coordinate delivery of high quality pharmaceutical services to outpatient infusion patients and hospital patients. Responsibilities
Enters, verifies and dispenses medication orders in accordance with scope of professional license and ensures proper documentation of drug use including ordering, storage and distribution of pharmaceuticals, including controlled substances. Reviews treatment plans and therapy plans for drugs used in the center (antineoplastics and other drugs) for clinical appropriateness, including correct dosing, drug interactions, and organ function adjustments. Evaluates lab results, other tests and vital signs to ensure safe medication administration. Collaborates with oncologists, providers, nurses and other care teams to build, review, and optimize treatment plans and therapy plans, making necessary adjustments as needed. Verifies and approves chemotherapy orders and regimens according to standard practice guidelines. Supervises pharmacy technicians in the aseptic compounding of chemotherapy and other sterile products per USP standards while ensuring compliance with state and federal laws, and GBMC policies and procedures. Maintains documentation for sterile compounding logs, investigational drug accountability and waste disposal. Performs final checks on compounded medications before dispensing. Organizes daily operations of the main infusion center and the satellite center(s). Closely reviews work of pharmacy interns and technicians, and when needed, performs the duties of a preceptor for pharmacy students. Advises physicians and nurses about drug interactions, incompatibilities, side effects, instructions for drug administration, and matters pertaining to the optimum use of medications. Resolves issues pertaining to medication use, clarifying orders and providing drug information between patient care areas and the Infusion Pharmacy. Performs Medical Use Evaluations (MUE) and facilitates safe, effective and economical use of drugs in a fiscally responsible manner. Identifies and anticipates issues, takes responsibility for correcting them with well-thought-out solutions. Participates in interviewing and hiring new employees, and oversees orientation and training of new and current employees. Mentors and trains colleagues, co-workers, newly hired pharmacists, pharmacy technicians, interns and students. Follows up on reporting of Adverse Drug Reactions and contributes to quality improvement activities in the department. Willingly performs assignments using critical thinking skills and collaborates with care teams to achieve optimal patient outcomes. Qualifications
Pharm D from an accredited college or university or BS in Pharmacy from an accredited pharmacy school with 0-3 years of experience. Current Maryland State Pharmacist License. Oncology Certified Pharmacist (BCOP) preferred. Thorough knowledge of current theory and practice of proper pharmacy services. Knowledge of oncology pharmacology, pharmacokinetics, adverse drug reactions and drug interactions and ability to interpret clinical laboratory data. Skill in sterile compounding and safe handling of hazardous drugs including chemotherapeutic agents. Familiarity with oncology guidelines such as NCCN and ASCO. Skill in data analysis and interpretation. Ability to organize and prioritize day-to-day activities to be most efficient when assigned to specific patient care units. Skill in problem assessment and resolution and ability to coordinate the work of others when tasked. Knowledge of personnel practices such as monitoring attendance, evaluating and improving performance, and handling employee complaints. Skill in managing teams and in oral and written communication. Good critical thinking skills. Pay Range & Benefits
Pay Range: $49.15 - $86.50. Final salary offer based on qualifications, education, experience and alignment with organizational needs.
