Join our team as a Case Manager for the Youth Kauhale Program, where you'll play a key role in supporting young adults experiencing homelessness on their journey to independence. You'll manage cases, coordinate with a diverse team, and ensure participants receive the services they need, including housing, medical care, and employment resources. Conduct assessments and develop service plans for participants Coordinate with a diverse team to provide housing, medical care, and employment resources Advocate for clients in meetings and connect them to vital community resources Manage cases to support participants on their path to independence Qualifications
Bachelor\'s Degree in Social Work, Psychology, or related Human Services field At least one (1) year of experience with youth and youth services Requirements
Exceptional verbal and written communication skills Proficiency in navigating electronic health record systems Consistent and timely documentation Access to a vehicle for transporting youth A valid driver\'s license Current auto insurance and safety check Benefits
