Our Mission
APluscare Behavioral Health serves individuals, groups, and communities with special needs by providing comprehensive care and individualized support services that maximize independence, empowerment, personal growth and quality of life. We provide care and support services in personal care, behavioral support, recreational and employment services. We are looking for kindhearted staff willing to go above and beyond in supporting our clients.
Responsibilities And Duties
Coordinate all necessary team meetings for the program. Develop, modify, and/or recommend amendments to service plans for Support Coordinators/Case Managers. Demonstrate knowledge of Division of Developmental Disabilities regulations, circulars, and licensing standards. Maintain service programs compliance with DDD, DHS, and other state departments as well as federal rules and regulations. Lead and guide program staff to assess clients\' strengths and needs in development areas through observation and assessment tools, in accordance with APluscare guidelines. Suspend or supervise support and service program supervisors to facilitate positive outcomes for clients served. Promote and model an appropriate support system and activities for clients served. Maintain full adherence to APluscare Policies and Procedures and to relevant service program manuals. Conduct weekly, monthly, and quarterly program audits to ensure quality supports and services for clients. Develop, distribute as appropriate, and monitor staff schedules. Coordinate planning and implementation of care and support plans, and assume responsibility for implementation and documentation as needed. Provide the Program Director with regular updates and reports on all clients. Prepare and submit monthly reports to the Program Director. Qualifications And Skills
A bachelor\'s degree Ability to cooperate with the licensee and department staff in inspections or investigations Successfully complete and demonstrate proficiency in all required training areas Valid Driver\'s License; must be at least 18 years of age Push, pull, and lift up to 50 lbs Benefits
