SFDPH strives to achieve its mission through the work of multiple divisions - the San Francisco Health Network, Population Health, Behavioral Health Services, and Administration. The mission of SFDPH's Jail Health Services (JHS) is to provide comprehensive, compassionate, and respectful care for incarcerated people while mitigating the effects of structural racism and historical injustices, as a fully integrated and collaborative member of San Francisco’s Public Health Department. Under the direction of the Deputy Director of Jail Health Services, the Director, Jail Health Care Coordination will provide strategic and operational leadership for all release planning and care coordination services delivered to individuals incarcerated in the San Francisco County Jails. This position is essential to meet the expanded responsibilities required under the CalAIM Justice-Involved Initiative, which mandates that correctional health programs deliver prerelease, Medi-Cal billable care coordination services. In close collaboration with Jail Health leadership and external partners, this role will ensure that all eligible individuals receive comprehensive reentry planning services, including needs assessments, collaborative care planning, behavioral health linkage, referrals to Enhanced Care Management and Community Supports, and warm hand-offs to community providers. The Manager, Jail Care Coordination (0923 Manager II) performs the following essential job functions: Oversees delivery of jail health care coordination and release planning services for incarcerated individuals, ensuring continuity of care and alignment with Medi-Cal billing requirements Supervises and supports a multidisciplinary team of care coordinators, ensuring high-quality, person-centered service delivery and staff development Monitors Medi-Cal billing operations for pre-release and reentry services including eligibility, registration and billing error management, audits and related data and reporting Ensures program compliance, data tracking, and quality improvement to meet CalAIM standards and maximize Medi-Cal revenue Makes recommendations regarding standards of work, policies, operations, and performance monitoring tools that support integrated, equitable, and accountable care coordination services in the jail setting. Leads interagency collaboration with managed care plans, behavioral health providers, community organizations, and criminal justice partners to support seamless pre-release and reentry service delivery Required Minimum Qualifications: Education:
Possession of a baccalaureate degree from an accredited college or university
AND Experience : Five (5) years of professional experience providing, monitoring, or coordinating medical or clinical social services Education Substitution : Additional qualifying experience may substitute for the degree requirement on a year-for-year basis for up to two (2) years.
