PACE (Program of All-Inclusive Care for the Elderly) was developed to support frail seniors and their families by delivering care in the community whenever possible. PACE participants receive in-home care services and transportation to a PACE center for primary medical care, social and recreational activities, and other senior care services. We are seeking leaders who share our mission to improve lives and advance healthcare. Position Summary:
Under the direction and supervision of the Regional Director of Program for All Inclusive Care of the Elderly (PACE) Operations, the PACE Center Director is responsible for the management of center operations — service delivery, quality of care, staffing and finances, and execution of program-related projects for the facility. The PACE Center Director serves as an effective link with On Lok's administration, OLSHS management team, and program staff to coordinate and execute all program-related tasks. Responsibilities
Program Leadership, Staffing Management & Development: Provides leadership in participant care planning, care coordination, risk management and service utilization. Oversees day-to-day center operations; ensures regulatory and compliance requirements are met. Maintains employee morale, provides ongoing training and mentoring, improves quality, productivity, and reduces turnover; sets standards for performance and behavior. Ensures quality, compassionate service delivery across On Lok external partnerships; strengthens systems, workflows, policies, programs, and performance; builds key partnerships across On Lok PACE centers and related settings. Acts as a liaison with On Lok internal and external partners, management teams, and program staff through regular communication and collaboration. Ensures adequate staffing and continuity of services; collaborates on staffing plans, supervises and evaluates staff performance, and promotes staff development; coordinates active participation of the interdisciplinary team (IDT) in care planning. Monitors staffing trends including performance, turnover, overtime, and recognition. Quality Assurance, Improvement & Compliance
Collaborates with Quality Assurance, Practice Management, Medical and program leadership to improve workflow efficiency and participant care outcomes. Implements on-site compliance monitoring and evaluation; develops corrective action plans as needed. Participant Care Management
Leads the IDT and center resources to develop, coordinate, monitor and execute participant care plan goals for all center participants. Provides resources, trainings, and tools to staff to deliver care planned goals. Identifies opportunities for care plan innovation and new service delivery options to separate On Lok PACE from other providers. Leads process improvements and trainings to enhance collaboration across departments; ensures high-quality IDT service delivery. Partners with other PACE Center Directors to standardize processes and drive improvements; strengthens cross-functional partnerships to leverage advanced technology for efficient staff use and communication with participants, customers, vendors and partners. Monitors compliance with regulatory standards on care planning and manages resources to avoid overprovisioning. Financials & Asset Management
Manages resources and systems to meet operational, human resources and budgetary goals. Oversees center finances, ensuring capital expenditures, operational expenses, staffing, and care utilization stay within budget. Oversees facility assets, space, equipment maintenance, and safety. Strategic Planning & Project Execution
Develops organizational goals, performance targets, and quality improvement plans; monitors financial projections. Involves in community promotion of On Lok; partners with Marketing, Outreach, and PACEPartners to enhance outreach and visibility. Leads standardization and continuous process improvement with change management; utilizes Lean Six Sigma and project management tools as appropriate. Other duties as assigned. Qualifications (Knowledge, Skills, Abilities)
Bachelor's Degree in Healthcare or related field 4-7 years of experience with geriatric or frail populations 2-4 years in healthcare-related operations and administration 2-4 years of financial management experience Demonstrated leadership and supervisory experience Strong facilitation, communication and interpersonal skills Ability to bridge health and social issues in daily operations Proven ability to formulate and execute business goals and objectives Proficiency with standard business software, EHR, enterprise systems and Microsoft Suite The above statements describe the general nature of work performed.
