The listed minimum pay reflects compensation for a full-time equivalent (1.0 FTE) position. Actual compensation may differ depending on hours and status (e.g., part-time). Minimum Pay: $94,140.80

Position overview

Under the supervision of and in partnership with the Executive Director, manages clinic flow and provides supervision of PACE clinical services to ensure high-quality standards. Assists Medical Director with clinical and operational duties and special projects as required. Coordinates services with the primary care team and serves as the primary liaison between clinical support and the Interdisciplinary Team (IDT). Collaborates with PACE Primary Care Providers to ensure thorough and reliable clinical support processes. Ensures proper staffing to meet clinical care needs of PACE participants within the clinic and partner facilities. Provides skilled nursing care, implements orders, develops care plans, and educates participants, caregivers, families, and staff; communicates participants condition to caregivers, primary care, and the IDT.

Essential functions Manages and oversees nursing and clinical support staff (RNs, Home Care RNs, MAs, Schedulers, Medical Records/HIM), including coaching, education, training/observations, reporting, and competency evaluation.

Coordinates and supervises daily administrative needs of the clinical support team; organizes care plans, training, direction, and collaboration; provides clinical support decision making as needed.

Conducts audits and works with regulatory agencies and Quality Manager to identify trends.

Participates in Interdisciplinary Team meetings; communicates participant changes and coordinates 24-hour care delivery.

Coaches and mentors staff; ensures practice within scope; fosters teamwork and provides feedback and redirection as needed.

Defines and communicates job responsibilities and accountability; conducts performance evaluations and corrective action oversight.

Communicates with Center Director and Medical Director to align practices across the organization.

Evaluates clinic effectiveness and drives quality improvement; delegates process improvement tasks as needed.

Ensures efficient clinic flow; monitors appointment utilization; assists PACE team in triage and addressing acute needs.

Designee to communicate with IDT and community agencies; oversees orders, medications, and dissemination of instructions.

Coordinates participant care with outside providers – nursing homes, assisted living, lab, oxygen, etc.; supports weekend and after-hour staff; maintains on-call coverage calendar.

Assumes role of clinic nurse to provide direct patient care when needed; may assist in off-site assessments depending on staffing.

Leads clinical team in emergency/advanced care situations; collaborates with Center Director on expectations; adjusts workflows by acuity and census.

Partners with Center Director and Executive Director to forecast staffing and make fiscally responsible supply and equipment decisions.

Participates in continuing education and required staff meetings; acts within authority to practice.

Minimum requirements Education: Bachelor’s degree in Nursing required; Master’s preferred.

Experience: Five years of clinical long-term care experience (including one year with frail elderly population) and five years of supervisory experience.

Other credentials: AHA BLS. Non-Licensed must obtain before end of orientation period. Driver’s license; RN license in NJ or multistate.

Knowledge and skills: Ability to establish and maintain effective working relationships with participants, co-workers, medical staff, and stakeholders; professional demeanor with all.

Special training: Experience with clinical charts/records; proficient with Microsoft Office and general computer skills.

Mental/behavioral: Strong organizational and supervisory skills; ability to handle difficult situations; ability to build interpersonal relationships with staff and participants.

Physical demands and work environment Frequent: Standing, walking, carrying, pushing/pulling, bending, reaching, overhead reaching, squatting/kneeling, wrist movements, fine motor tasks.

Occasional: Sitting, climbing, keyboard use/repetitive motion.

Continuous: Lifting 30 lbs from floor to waist and above 30 lbs; sensory requirements include near/far vision, color discrimination, depth perception; hearing requirements.

Exposure risks: Airborne diseases, noise, dust/particulate matter.

Pay information: The listed rate is a good-faith estimate and may be adjusted. Bonuses and incentive eligibility depend on role and level.
