Acclaim Multispecialty Group is seeking a physician assistant or nurse practitioner to provide outpatient assessment and treatment for adult and pediatric patients in the Health Connect Clinic. The role focuses on timely access and connection, ensuring patients are linked to appropriate primary care, behavioral health, or specialty services for ongoing management. Responsibilities include developing short-term care plans, coordinating diagnostics, treatment, and follow-up, and collaborating with multidisciplinary teams to optimize transitions. The provider will deliver patient-centered care, support continuity and patient satisfaction, and participate in quality improvement and clinical performance programs. The Health Connect Clinic serves as an access point for patients needing timely connection to primary care, behavioral health, and other essential services. Responsibilities include: Provide outpatient assessment and treatment for adult and pediatric patients Ensure timely access and connection to primary care, behavioral health, or specialty services Coordinate diagnostics, treatment, and follow-up Collaborate with physicians, residents, advanced practice professionals, medical students, care coordinators, social workers, and other teams Utilize national evidence-based guidelines and JPS/Acclaim protocols Demonstrate compassionate, patient-centered care and support continuity Maintain relationships with community and clinic stakeholders Participate in quality improvement, peer review, and clinical performance programs Represent the Health Connect Clinic and JPS Health Network with patients and families Performs other job-related duties as assigned Knowledge, Skills And Abilities
Comfort working in a large public/county facility Working knowledge in the assigned area, with competency in assessment, diagnosis, and treatment Strong interpersonal, verbal, and written communication skills Self-directed with initiative to engage in community activities and medical outreach Benefits
