Job Summary The JPS Health Network Health Connect Clinic serves as an access point for patients who need timely connection to primary care, behavioral health, and other essential services. Providers deliver short-term outpatient care, addressing immediate needs while ensuring patients are transitioned to the most appropriate ongoing care setting. This role supports the mission of JPS Health Network by improving access, continuity, and coordination of care for the community.

Duties and Essential Job Functions Provides outpatient assessment and treatment for adult and pediatric patients in the Health Connect Clinic. Focuses on timely access and connection, ensuring patients are linked to appropriate primary care, behavioral health, or specialty services for ongoing management. Addresses immediate health needs, develops short-term care plans, and coordinates necessary diagnostics, treatment, and follow-up. Collaborates with multidisciplinary teams, including behavioral health providers, care coordinators, social workers, and primary care teams, to optimize transitions. Utilizes national evidence-based guidelines and JPS Health Network/Acclaim Multispecialty Group protocols to guide clinical decisions. Demonstrates compassionate, patient-centered care that supports continuity and patient satisfaction. Maintains strong collaborative relationships with physicians, residents, advanced practice professionals, medical students, and staff. Participates in quality improvement, peer review, and clinical performance programs within the JPS Health Network. Serves as a resource to the community by helping patients navigate healthcare access and support services. Represents the Health Connect Clinic and JPS Health Network through professional engagement with patients, families, and community stakeholders. Performs other job-related duties as assigned.

Knowledge, Skills and Abilities

Must be comfortable working in a large public/county facility

Must be able to demonstrate a working knowledge in area assigned, and competency in assessment techniques, diagnosis and treatment processes that apply to the assigned medical specialty area

Must have good interpersonal, verbal and oral communication skills to work effectively with other health care professionals and serve as a resource to the community

Must be self-directed and take initiative to develop and participate in community activities and medical outreach programs pertaining to the medical specialty area

Qualifications

Graduate of an accredited Nurse Practitioner program with an active Texas License

Must have current Texas APRN license with ACNP certification

OR

Graduate of an accredited Physician Assistant Program with an active Texas License

Required Licensure/Certification/Specialized Training

Current, unrestricted medical license to practice in the State of Texas

Unrestricted work authorization to work in the United States
