• Estimated Weekly Salary Range: $1870-$1920

Qualifications

• Active Registered Nurse (RN) license – New Mexico or compact state
• Minimum 1 year of recent home health nursing experience (within the past 3 years)
• Proficiency with HCHB and OASIS EMR systems
• Strong clinical assessment and documentation skills
• Excellent communication and organizational abilities
• Ability to work independently in a field-based home health role

Responsibilities

• Deliver skilled nursing care to patients in their homes throughout Albuquerque and surrounding areas
• Perform comprehensive patient assessments and create individualized care plans
• Accurately document patient care using HCHB and OASIS EMR systems
• Collaborate with interdisciplinary healthcare team members to achieve optimal patient outcomes
• Educate patients and families on disease management, medications, and self-care techniques
• Ensure compliance with agency policies and state/federal regulatory standards
