Position Summary:
The Adult Mental Health Residential Care Manager works as part of a collaborative team to support the day-to-day care and coordination of services for adults living in a residential treatment setting. This position plays a key role in ensuring residents receive timely medical and behavioral health care, participate actively in their treatment, and live in a safe, supportive, and recovery-oriented environment. The Care Manager contributes to treatment planning, coordinates external appointments, and provides direct care coverage as needed.
Key Responsibilities: Collaborate with the treatment team to develop, implement, and monitor individualized service plans. Coordinate and schedule medical, psychiatric, and other clinical appointments for residents; ensure transportation and documentation are in place. Maintain ongoing communication with healthcare providers, pharmacies, and other external supports to ensure continuity of care. Participate in regular treatment team meetings and contribute to care planning and progress reviews. Maintain residents' treatment plans as necessary Maintain accurate, timely documentation in accordance with agency, regulatory, and clinical standards. Assist residents in managing daily living activities and developing independent living skills. Support residents in adhering to medication schedules and treatment recommendations. Provide direct care coverage as needed, including during staff shortages, emergencies, or high-acuity situations. Collaborate closely with other Residential Care Managers, Nursing staff, Program Supervisors, and the Program Director to maintain a consistent, supportive environment. Promote a trauma-informed, person-centered approach to care. Qualifications:
Education: Bachelor's preferred Experience: Minimum of 1-2 years working in a residential, behavioral health, or community-based setting preferred. Strong communication, organizational, and problem-solving skills. Ability to work both independently and as part of a collaborative, multidisciplinary team. Familiarity with mental health diagnoses, recovery principles, and community support services. Proficiency with electronic health records and documentation practices. Requirements:
Minimum of a High School Diploma or GED Professionalism and a commitment to excellence in care Must have and maintain a valid driver's license, use of an insured vehicle and an acceptable driving record Preferred:
Bachelor's Degree in Human Services or Psychology. At least 2 years of experience in mental services with adults Additional Information:
Base Rate: $19.23/ Hourly Schedule: Day shift Bilingual-Spanish differential: An additional $1.50 added to the base rate if Spanish proficient We provide:
