JUSTICE RESOURCE INSTITUTE (JRI) seeks a detailed-oriented Long-Term Services and Support (LTSS) Supervisor. The LTSS Supervisor has a flexible schedule working as a Part-Time Care Coordinator with a caseload of clients and as a Part-Time Supervisor.

The LTSS Supervisor provides team oversight of care coordination and clinical care management for MassHealth Members with complex medical and behavioral health needs who are enrolled in an Accountable Care Organization (ACO) or Managed Care Organization (MCO) plan. The Supervisor collaborates with the Community Partner team and the clinical staff of each Enrollee's ACO/MCO's plan to minimize duplicative efforts, promote integrated care, ensure quality and continuity of care, and support the values of person-centered planning. The Supervisor is at the helm of organizing and coordinating resources and services in response to the Enrollees' healthcare needs across multiple settings, and inclusive of both LTSS and SDH needs. This role drives assessment and care planning, care transitions, health and wellness coaching, as well as community and social services connections across a multi-disciplinary care team for CCP enrollees.

JRI provides the training so that: YOU will be able to develop and utilize your clinical skills, managerial skills and overall leadership skills to guide effective trauma-based treatment. YOU will be able to develop and utilize your leadership skills by providing complete oversight of programming including oversight of the operations of all client services and supports set forth by licensing and regulatory partners. YOU will be able to ensure that all staff are working toward a shared mission and vision of the program and provide direct supervision to departmental leaders/Administrators. REQUIREMENTS:

A Master's Degree in social work, human services, psychology, sociology, or related field is required. Clinical licensure (LICSW, LCSW, LMHC) is required. Applicants must have at least five years of experience working with pediatric populations, with at least three years supervisory experience required. Applicants should have excellent communication skills (oral and written) and demonstrated leadership abilities. Experience or knowledge of social service organizations and health care providers for youths with Long-Term Services and Support Needs preferred. Excellent collaboration and communication skills with the ability to partner effectively across the organization, key provider partners, and ACO/MCO plans as well as state agencies to build a robust LTSS capacity. Demonstrated ability to establish and manage performance and outcome metrics. Excellent organizational, time-management, and problem-solving skills. Applicants must be knowledgeable in growth and development; able to obtain and interpret information in terms of the needs of complex medical and behavioral health needs, intellectual and developmental disabilities and their families; and understand the range of interventions and treatments required by this population. Applicants must maintain a valid driver's license, as driving is a requirement for this position using a personal vehicle. Available to flexible scheduling to support families and staff as needed.
