Overview Long-Term Services and Support (LTSS) Supervisor. The LTSS Supervisor provides team oversight of care coordination and clinical care management for MassHealth Members with complex medical and behavioral health needs who are enrolled in an Accountable Care Organization (ACO) or Managed Care Organization (MCO) plan. The Supervisor collaborates with the Community Partner team and the clinical staff of each Enrollee’s ACO/MCO plan to minimize duplicative efforts, promote integrated care, ensure quality and continuity of care, and support the values of person-centered planning. The Supervisor organizes and coordinates resources and services in response to enrollees’ healthcare needs across multiple settings, including LTSS and social determinants of health (SDH) needs. This role drives assessment and care planning, care transitions, health and wellness coaching, and community and social services connections across a multidisciplinary care team for CCP enrollees.

Responsibilities

Provide team oversight of care coordination and clinical care management for MassHealth Members with complex needs in ACO/MCO plans.

Collaborate with Community Partner teams and ACO/MCO clinical staff to promote integrated care, avoid duplicative efforts, and uphold person-centered planning values.

Organize and coordinate resources and services across LTSS and SDH needs to respond to enrollees’ healthcare requirements.

Lead assessment and care planning, care transitions, health and wellness coaching, and connections to community and social services within a multidisciplinary team for CCP enrollees.

Requirements

Master's Degree in social work, human services, psychology, sociology, or related field.

Clinical licensure (LICSW, LCSW, LMHC) required.

At least five years of experience with pediatric populations, including at least three years of supervisory experience.

Excellent oral and written communication skills and demonstrated leadership abilities.

Experience or knowledge of social service organizations and health care providers for youths with LTSS needs preferred.

Strong collaboration and communication skills to partner across the organization, provider partners, ACO/MCO plans, and state agencies to build LTSS capacity.

Ability to establish and manage performance and outcome metrics.

Excellent organizational, time-management, and problem-solving skills.

Knowledge of growth and development; ability to interpret information related to complex medical/behavioral health needs and developmental disabilities; understanding required interventions and treatments.

Valid driver’s license and willingness to use a personal vehicle for work.

Flexible scheduling to support families and staff as needed.

Background and driving record checks will be performed.
