The Case Manager will be responsible for the full client lifecycle, from admitting new clients and determining their requirements to establishing treatment programs, monitoring cases, and preparing for discharge. This role involves advocating for client services, maintaining records, and improving treatment results by evaluating and redesigning processes. Position Responsibilities
Admit new clients by reviewing records and applications and conducting orientations. Determine client requirements through intake interviews and evaluation reviews. Establish and coordinate treatment programs, schedules, and resources. Monitor cases by verifying attendance, evaluating treatments, and observing responses. Advocate for needed services, intervene in crises, and provide support to clients and families. Maintain client records, log events and progress, and communicate progress to the therapeutic team. Prepare client discharge plans and coordinate post-discharge requirements. Prepare reports by collecting, analyzing, and summarizing treatment data and trends. Position Qualifications
2-4 years of experience in case management or a related social services role. Experience in client intake, interviews, and needs assessment. Ability to establish and coordinate treatment programs and services. Strong skills in case monitoring, client advocacy, and crisis intervention.
