Bilingual Care Navigator in Chicago/O'Hare
LHH is hiring a dedicated Care Navigator for our client near O'Hare. The person in this role will be performing case management duties, working with pregnant women and women with infants. Additional responsibilities includes community outreach and referrals to available resources.
Job overview: Hiring type: Contract to Hire Location: This position is based out of the O'Hare office (free parking garage) and will eventually travel to meet clients in their designated zones in Chicago; reliable transportation is required Hours: Monday - Friday, 830AM-5PM Hourly pay rate: $21/hour + mileage reimbursement

Summary, • Supporting pregnant women and their infants via phone or in-person meetings • Conduct initial assessment and navigation to link participants to available services, including medical, social, financial, housing, food access, or others • Educating participants on maintaining prenatal and preventative healthcare practices, safe infant sleep practices, infant and maternal nutrition, and milestones for infants • Community outreach and engagement events • Documenting participant meetings and case notes
Qualifications, • Associate degree in social services or related field required; Bachelor degree preferred
• Bilingual language skills (English and any other language - Spanish, Russian, Polish, etc.) • 6 months or more of experience working in the social services field, specifically with women and children • Must have reliable transportation and willingness to meet clients in-person • Strong interpersonal and relationship-building skills • Compassion and participant advocacy skills
Pay Details:
$21.00 per hour
