Lead and Innovate: Take the helm in developing cutting-edge tools, processes, and procedures to prioritize and review discharge medication reconciliations. Enhance Patient Outcomes: Partner with our Medication History Technician Supervisor to improve the quality and consistency of admission medication history collection, ensuring a seamless patient experience. The Clinical Pharmacist Specialist reports to Director Pharmacy Services, and indirectly to a Senior Clinical Pharmacist Specialist. Coordinates pharmacy clinical programs, collaborates with multidisciplinary care givers to monitor, assess, and optimize medication use across therapeutic areas, and serves as a subject matter expert in the area of specialty. Participates in policy development, quality improvement, and budget planning for Pharmacy Services and clinical practice settings. Minimum Qualifications
BASIC KNOWLEDGE : Bachelor of Science in Pharmacy or Doctor of Pharmacy degree from an accredited college of pharmacy. Advanced study in pharmacy or health care related field, preferably a master's degree or comparable work experience. Must be a Registered Pharmacist in the State of Rhode Island. Maintains current unrestricted licensure as a licensed pharmacist in Rhode Island. Maintains current American Heart Association BLS and ACLS Certifications for emergency response as assigned. Demonstrates knowledge to provide care across age groups (neonate to geriatric). Experience
Post Graduate Year 2 (PGY-2) Residency in area of specialty or national board certification in area of specialty preferred; MTM certification. Clinical experience and knowledge may be considered in lieu of formal training. Strong interpersonal and communication skills; organizational and presentation skills. Knowledge of pharmacy technology and information systems; proficient in MS Word, Excel, Access, Outlook, and web browsers. Work Environment And Physical Requirements
Extended periods of standing/walking. Visual and manual dexterity for computer use. Occasional travel between facilities/sites. Independent Action
Functions independently within a broad scope of departmental policies and practices. Refers complex problems to Senior Clinical Pharmacist as needed. Supervisory Responsibility
