CONSUMER SERVICE COORDINATOR SELF DETERMINATION PROGRAM (SDP) LEAD TRAINING SPECIALIST NLACRC is a private, non-profit agency contracted with the State of California, Department of Developmental Services, to provide services to individuals with intellectual and developmental disabilities. NLACRC serves the San Fernando Valley, Santa Clarita Valley, and Antelope Valley since 1974, across the lifespan from infants to aging adults. Scope : Provides information, advocacy, and case management stabilization services for consumer participants of the Self-Determination Program (SDP). Supervision : Receives general supervision from Consumer Services Supervisor/Manager. Responsibilities

Establish the individual budget, spending plan, and case management services for consumers registering and/or participating in SDP, including coordination and facilitation of individual program planning to identify SDP-specific outcomes and supports. Assist participants with the transition in and out of the SDP. Assist in transferring funds between budget categories as identified in statute and/or regulation. If selected as an Independent Facilitator by the participant, assist in the person-centered planning process and the development of the SDP-based IPP. Participate in decision-making related to the development of the individual budget, assessment of short- and long-term needs, and coordination of participant-selected services and supports as identified in the IPP. Document and complete forms and reports in accordance with regulations and NLACRC policies and procedures. Work with Independent Facilitators and Financial Management Services providers for person-centered planning, payments, and related services. Initiate and respond to communications with consumers, families, service providers, and outside agencies on SDP-related service eligibility and problem resolution. Schedule and attend interdisciplinary meetings with individuals served, family members, service providers, advocates, etc. May require re-training staff on identified areas. Shadow and coach CSCs in the field during planning and conducting meetings. Assist in addressing issues related to service delivery and SDP program planning. May perform other tasks to ensure timelines, performance measures, and customer service goals are met. Identify barriers to service delivery and implement culturally sensitive strategies that embody empathy, trust, and respect; comfortable working in underserved and non-traditional settings to meet community needs. Employment Guidelines

Education & Experience

Bachelor’s degree in psychology, social work, sociology, or related human services field with two years of related experience. A Master’s degree may be substituted for experience. This position is non-exempt and may be located in the San Fernando, Santa Clarita, and/or Antelope Valley offices, and requires travel to all offices and throughout and outside of the catchment area as appropriate. Skills and Abilities

Understanding of family systems and appreciation for person-centered planning; empowering individuals to achieve their goals. Knowledge of intellectual and developmental disabilities; strong organizational skills; ability to work independently and as part of an interdisciplinary team; excellent verbal and written communication skills; proficiency with MS Word, MS Excel, and MS Outlook; proficiency with basic bookkeeping and budgeting concepts. 3 years of NLACRC Regional Center experience/Consumer Service Coordinator preferred. Essential Requirements

Service coordination is a community-based role requiring meetings with individuals served and their families in homes, communities, or offices. Must be comfortable working with a high degree of independence. Frequent telephone, virtual, and out-of-office meetings required. Valid California Driver’s License and reliable transportation, or acceptable substitute, required. Benefits and Development

Effective March 2025, pay range is $29.49 - $41.66/hour (internal Grade 8). Base Pay Rate / Salary Range Information The actual amount offered will depend on qualifications, degree, experience, skills, and other factors.
