Braintree Primary Care Network (PCN) Ltd is seeking a motivated andexperienced Advanced Clinical Practitioner to join our multidisciplinary team.The successful candidate will play a key role providing support across ourthree member practices and two hosted practices, serving a combined populationof approximately 58,000 patients. The post holder willbe practising as an Advanced Clinical Practitioner (ACP) within a primary care hub. The role will encompass the skills ofassessment, examination, diagnosis, and treatment within an agreed scope ofpractice. The post holder will assist in the safe assessment, treatment,referral, and discharge of patients in their designated directorate. Main duties of the job

The post holder will - Worktowards health promotion and prevention and comprehensively assess patients forrisk factors and early signs of illness. Draw on a diverserange of knowledge in their decision-making to determine evidence-basedtherapeutic interventions, which will include prescribing medication wherelegally allowed and actively monitoring the effectiveness of therapeuticinterventions. Plan and managecomplete episodes of care, working in partnership with others, delegating andreferring as appropriate to optimise health outcomes and resource use, andprovide direct support to patients and clients. Jobsummary The post holder will be practising as an Advanced Clinical Practitioner(ACP) within their designated specialty/directorate. The role will encompassthe skills of assessment, examination, diagnosis, and treatment within anagreed scope of practice. The post holder will assist in the safe assessment,treatment, referral, and discharge of patients in their designated directorate. His or her practice should be developed to an advanced level wherebythe Advanced Clinical Practitioner can directly or indirectly influence allaspects of care and management of patients within their speciality, and acrossa population. The post will bestructured around the 4 pillars of advanced practice, available time allocatedas indicated in brackets: Expert clinicalpractice ProfessionalLeadership and consultancy Education, training,and development Practice and servicedevelopment, research, and evaluation Clinical Uses specialistknowledge and specific training to act autonomously in assessing, planning, andimplementing a comprehensive management plan within their defined scope ofpractice. Elicit a patienthistory appropriate to the clinical situation, which will include, presentingcomplaint, history of the present illness, past medical history, socialhistory, family history, medications, allergies, review of systems, riskfactors and appropriate targeted history. Perform a physicalexamination tailored to the needs of the patients and the demands of theclinical situation. Assess and prioritisepatients undergoing treatment. Demonstrate continualevaluation of the patients and use expertise to recommendadjustments/amendments to treatment plans, in consultation with the patient andmembers of the multi-professional team. Make direct referralsto other members of the health care team. Requests bloods andradiological test as appropriate and within agreed scope Request and performdiagnostic tests as agreed locally and within agreed scope. Chase, view andInterpret results and report findings. Discuss and agreeassessment outcomes with patients, carers, and other health care professionalsto enable patients to make an informed decision regarding their treatment. Communicate highlycomplex information to patients, carers and families during the consent processprior to commencement of treatment plans and invasive procedures includingexplaining treatment options. Actively supportspatients and their families through communicating difficult and highly complexsensitive information using a variety of methods to ensure effectiveunderstanding in a timely manner. Proactively supportspatients, carers, and families in coming to terms with their illness/condition. Provides empathy andreassurance through understanding the quality-of-life issues associated withtheir illness/conditions and treatment related side effects. Ensure that accurate,essential, and appropriate written and verbal information is conveyed to thewider MDT to ensure effective management of patients. Ensure dignity,privacy and cultural and religious beliefs are respected at all times. Adheres toprofessional code of conduct, works within the boundaries of their scope ofpractice and always manage associated clinical risks effectively. Professional

leadership and Management Act as a role model by demonstrating high standards of holistic care. Act in such a way as to be a credible, effective leader, demonstratingeffective clinical leadership on a daily basis. Take key responsibility for supporting the wider MDT. Effectively manage own dairy and workload. Ensure that documentation is of a very high standard reflective ofAdvanced Clinical Practice, adhering to local and national guidelines. Identify the skills set and terms of reference required of the AdvancedClinical Practitioner role, ensuring they reflect the individual, holisticneeds of patients undergoing care. Works in collaboration with the MDT to investigate clinical incidentsassociated with patients in their care. Assess and monitor risk in own and others practice, acting on results,thereby ensuring safe delivery of care. Diffuse potential complaints and hostile situations with staff,patients and carers using highly developed negotiation and interprofessionalskills Provide representation, as appropriate at various local and nationalmeetings, providing feedback to the organisation on clinical and professionalissues which have an impact on care and standards of practice within theirsphere of responsibility. Develop formal andinformal links outside the organisation, sharing good practice, innovativeideas, and promote staff and service development. Establish and maintaina regional and national network of contacts relevant to the service. Promote the servicethrough formal and informal presentations within and outside the organisationthrough study days, conferences, and written papers. Education and Training Has obtained thetheoretical knowledge; MSc in advanced practice or equivalent, in conjunctionwith practical experience; evidenced in a supporting portfolio, to a level thatallows the job responsibilities to be delivered at a high standard. Has obtainedNon-Medical Prescribing if appropriate to registered profession. Identify own personaldevelopmental and educational needs to work at an advanced level and beyond.Ensure appropriate action is taken to maintain and further develop skills. Maintain aprofessional portfolio and participate in continuing professional development. Maintain competenciesand attend mandatory lectures and training as required by the PCN. Receive clinicalsupervision to clinically improve knowledge and the quality of care deliveredto patients. Utilises professionalknowledge and skills, underpinned by theoretical and relevant practicalexperience to teach, motivate, and support junior staff on a range of clinicalpractices. Participates in thedelivery of specialist training and development of other healthcare workerswithin the PCN. Develop and organisestudy days and training sessions for self and others within the clinical team. Participates in thesupervision and mentorship of multi-professional students and junior staff. Undertake clinicalsupervision for trainees. Carry out trainingneeds analysis, using the results to design, develop and deliver a teachingprogramme promoting practice development for those staff caring for patientswithin the speciality. This will include the development of links with otherorganisations and innovative approaches to staff development. Research, service development and audit Maintain up to dateknowledge in the specialist field, using information to affect change inpractice and ensuring effective dissemination of new knowledge. Participate in servicedevelopment. Writes protocols, guidelines, and procedures for own remit which,impacts on other members of the multi-disciplinary team. Participate in thedevelopment and evaluation of protocols and guidelines and procedures usingcurrent literature and research. Participates in thedevelopment of patient information to improve patient care. Liaise with MDT,including consultants, senior nursing colleagues and management in thedevelopment of services, contribute ideas and make recommendations for serviceimprovements. Implement policiesinforming members of the multidisciplinary team of any changes. In collaboration withthe MDT ensure that clinical practice is patient centred and researched based,in accordance with professional practice, guidelines, and national and localbenchmarks. Support relevant auditactivity in the specialities to evaluate the effectiveness of careinterventions and disseminates outcomes with any potential changes to clinicalpractice which improves health outcomes. Regularly audits ownpractice. Where appropriateparticipate in research projects and/or support the wider MDT with researchinitiatives. Significant experience of working as a registered practitioner and within a relevant specialty [4-5 years Minimum] Evidence of recent teaching, mentorship, and supervision of staff Evidence of multi-professional working Evidence of audit experience Evidence of involvement in change management Qualifications

Professional Registration with either NMC, GPhC or HCPC. Evidence of continuing education Possession of 1st degree in a clinical field e.g., Nursing, physiotherapy at a minimum 2:2 classification MSc in Advanced Practice or equivalent (i.e., registered on Centre of advancing practice directory) Non-Medical Prescribing if appropriate to registered profession.
