Braintree Primary Care Network (PCN) Ltd is seeking a motivated andexperienced Advanced Clinical Practitioner to join our multidisciplinary team.The successful candidate will play a key role providing support across ourthree member practices and two hosted practices, serving a combined populationof approximately 58,000 patients.
The post holder willbe practising as an Advanced Clinical Practitioner (ACP) within a primary care hub. The role will encompass the skills ofassessment, examination, diagnosis, and treatment within an agreed scope ofpractice. The post holder will assist in the safe assessment, treatment,referral, and discharge of patients in their designated directorate.
