We are seeking to recruit a friendly, enthusiastic and self-motivated individual to join our professional and supportive team. We are looking for an experienced AHP/ ACP to work 24 hours a week, based at our GP practice in Abbeydale, Gloucester. Thesuccessful candidate will be expected to deliver a high standard of patientcare using autonomous clinical skills, and be an independent prescriber. You will be supporting patients that need urgent/ same day care. Main duties of the job

Our list size is approximately 19,500. The successful candidate will be expected to deliver a high standard of urgent care to patients using autonomous clinical skills, and be an independent prescriber. Contracted hours will ideally be in the region of 24 hours per week. You will provide a holistic approach to individuals including assessment, diagnosis, planning and implementation of high quality care. You will work within our urgent care team, alongside other AHPs/ ACP and GPs. We have effective processes and procedures for ensuring patients are seen by the right person, in the right time frame. We are a large GP practice, in Abbeydale, Gloucester. We offer a supportive, well-managed working environment and a happy and functional practice team and have received an extremely positive CQC inspection. We have a £5M building that helps us to meet the clinical needs of our patient population. We are a SystmOne practice, employing almost 90 staff. Our team consists of GPs, ANPs, Paramedic, Practice Nurses, Mental Health Practitioners, Physiotherapists, HCAs, Patient Advisors and Administrators. We also benefit from PCN staff including Social Prescribers and a Health and Wellbeing Coach. We are busy but we maintain a strong sense of one team working together to deliver excellent care to our patients. The post holder is an experienced nurse who,acting within their professional boundaries, will provide care for thepresenting patient from initial history taking, clinical assessment, diagnosis,treatment and evaluation of their care.They will demonstrate safe, clinical decision making and expert care forpatients within the general practice.They will work collaboratively with the multi-disciplinary generalpractice team to meet the needs of patients, supporting the delivery of policyand procedures and providing nurse leadership as required. In order to work at this level NMCrequirements for advanced practice must be met. Scope andPurpose of the Role To deliver a highstandard of patient care as Nurse Practitioner/Advanced ClinicalPractitioner in General Practice, using advanced autonomous clinicalskills and a broad and in-depth theoretical knowledge base. To manage aclinical caseload, dealing with presenting patient needs in a primary caresetting. To ensure thehighest standards of care are provided for the patients To develop theservices offered by the practice. JobDimensions To manage ownclinical workload in general practice responding effectively to patientneed and ensuring patient choice and ease of access to services. The developmentand use of referral pathways to the local acute NHS Trust services and toDiagnostic and Treatment Centres. To mentor andsupport other health care professionals in developing and maintainingclinical skills. Primaryduties & areas of responsibility CLINICALROLE: The post holder will: Work alongside theon-call GPs. Provide telephonetriage service and face ot face appointments for patients who request an on-the-day appointment,deciding on the most appropriate course of action for the patient. Provide a firstpoint of contact within the practice for patients presenting withundifferentiated, undiagnosed problems, making use of skills in historytaking, physical examination, problem-solving and clinical decisionmaking, to establish a diagnosis and management plan. Provide the choiceof direct access to a NP/ACP, both in the practice and over the telephonefor the general practice population. Makeprofessionally autonomous decisions for which he/she is accountable. Instigatenecessary appropriate invasive and non-invasive diagnostic tests orinvestigations and interpret findings/reports. To prescribe safe,effective and appropriate medication as defined by current legislativeframework. Provide safeevidence-based, cost effective, individualised patient care. Offer effectiveservice to patients and their families, developing where appropriate anon-going plan of care/support with an emphasis on prevention and self-care. Promote health andwell-being through the use of health promotion, health education,screening and therapeutic communication skills. Refer patientsdirectly to other services/agencies as appropriate. Work with nursing,medical and health care assistant colleagues to ensure that NationalService Frameworks (e.g. coronary heart disease, diabetes, mental health)are being delivered. Work withmulti-disciplinary teams both within the practice and external services topromote integrated and seamless pathways of care. Contribute to thepractice achieving its quality targets to sustain the high standards ofpatient care and service delivery. Participate in identificationof community health needs and develop patient/family centred strategies toaddress them. Help develop andset up new patient services and participate in initiatives to improveexisting patient services. Be aware of andcontribute to planning and delivery of GP commissioning. TEACHINGAND MENTORING ROLE: The post holder will: Promote a learningenvironment for patients, nurses and other health professionals. Contribute to theplanning and implementation of the teaching for existing staff, such asmedical students, foundation doctors, GP registrars, health careassistants within the practice. PROFESSIONALROLE: The post holder will: Promoteevidence-based practice through the use of the latest research-basedguidelines and the development of practice-based research Monitor theeffectiveness of their own clinical practice through the quality assurancestrategies such as the use of audit and peer review. Maintain theirprofessional registration. Participate incontinuing professional development opportunities to ensure thatup-to-date evidence-based knowledge and competence in all aspects of therole is maintained. Work within thelatest NMC Code of Professional Conduct. Record acuteconsultation data in patients records in accordance with the latest guidelines and other pertinent standards Keep up to datewith pertinent health-related policy and work with the practice team toconsider the impact and strategies for implementation. Workcollaboratively with colleagues. Demonstrate leadership. Pro-activelypromote the role of the NP/ACP within the practice and externally to keystakeholders and agencies. MANAGERIALROLE: The post holder will: Act as a nurseadvisor to other members of the Primary Healthcare team Encourage anddevelop teamwork within the practice Participate inmulti-disciplinary protocol and Patient Group Directions development Identifyappropriate opportunities to delegate both clinical and administrativetasks to a more junior staff Help the practiceoperate in a cost-effective manner Identify andmanage nursing care risks on a continuing basis Participate inpractice meetings, reporting progress as required Participate inaudits and inspections as appropriate Work alongsideother members of the Practice team in the setting up and/or improving ofpractice systems for monitoring/measuring performance against ClinicalGovernance and Quality indicator targets Ensure that allPractice Policys are fully implemented HEALTH& SAFETY/RISK MANAGEMENT: The post-holdermust comply at all times with the Practices Health & Safety policies,in particular by following agreed safe working procedures and reportingincidents using the organisations Incident Reporting System. Knowledge and Skills

Knowledge: - Understanding and knowledge of policy developments related to the delivery of Primary Care services including General Practice, the GMS contract, Clinical Governance, Quality Outcomes Framework - Understanding of systems to gain an understanding of the health needs of the Practice population as they relate to Primary Care. - Understanding of evidence based practice - Knowledge of national standards that inform practice (e.g. National Service Frameworks, NICE guidelines etc) - Understanding of their accountability arising from the NMC Code of Professional Conduct (2004) and medico-legal aspects of the Nurse Practitioner role - Understanding of equal opportunity and diversity issues Skills: - Ability to assess and manage patient risk effectively and safely - Well developed word processing/data collection/IT skills - Excellent interpersonal, verbal and written communication skills - Time management and ability to prioritise workload - Able to analyse data and information, drawing out implications for the individual patient/impact on care plan - Able to establish and maintain effective communication pathways within the organisation and other health care services - Experience of use of SystmOne medical software package - Proven record of effective use of networking and influencing skills Qualifications

Registered General Nurse (Currently registered with the Nursing & Midwifery Council) Recognised NP/ACP qualification at minimum of degree level or equivalent - Experience

Minimum of 5 years post registration experience Evidence of appropriate continuing professional development activity to maintain up-to-date knowledge and on-going competence in all aspects of the NP/ACP role Evidence of working autonomously and as part of a team Proven ability to evaluate the safety and effectiveness of their own clinical practice - Disclosure and Barring Service Check
