Our contributions to independence come from the clinical and non-clinical services we provide at our day centers. The Program Director, reporting to the Executive Director, oversees critical PACE (Program of All-inclusive Care for the Elderly) functions including nutrition, social work, day center, personal care services, scheduling, and participant experience. The Program Director is accountable for developing and maintaining a high-functioning Interdisciplinary Team (IDT). Responsibilities
Manage critical PACE functions including hiring, training, supervising, mentoring staff, and ensuring clear and effective workflows Develop, plan, and facilitate an effective IDT and other participant-care related meetings; ensure they are organized and engaging, and that follow-up items are documented and tracked Oversee staff within the reporting structure to drive quality care and ensure adherence to SOPs and key regulatory requirements Collaborate to achieve high participant outcomes with shared participant services, including home care, home health, transportation, central call center support, and utilization management Oversee the WelbeHealth program site, supplies and equipment management, including monitoring and coordinating ongoing maintenance and vendor oversight Qualifications
Bachelor's degree in a relevant field such as health admin, health care management, nursing administration, gerontology, social work, or business administration; five or more years of relevant management experience may be substituted; master’s degree preferred Professional experience in a relevant field such as nursing, social work, psychology, or recreational/occupational/physical therapy, with demonstrated competence in working with frail elderly living at home required Minimum of seven (7) years of experience, with at least three (3) in a supervisory capacity, managing a senior health care program, outpatient clinic, or healthcare facility Multi-disciplinary management experience (nursing, social work, rehab) Value-based care experience preferred Salary and Benefits
