Primary Care Provider, Nurse Practitioner / Physician Assistant - Full-Time | PACE Program at WelbeHealth. Make a difference delivering life-changing, community-based care to vulnerable seniors. Provide primary care services to elderly participants within the PACE program. Engage in value-based, holistic care planning and delivery. Conduct home visits as part of regular hours (8am-5pm, Monday–Friday) with rotating on-call duties a few nights or weekends per month (on-call by phone). Collaborate with interdisciplinary team to support patient needs and care coordination. Qualifications
Licensed Nurse Practitioner or Physician Assistant with at least two years of experience working with the elderly. Prescriptive authority, DEA registration, and eligibility to serve Medicare/Medi-Cal participants. Schedule and Compensation
Regular hours: 8am-5pm, Monday through Friday, including home visits. On-call: rotating schedule few nights or weekends per month, available by phone only. Competitive compensation: $145,399 - $174,478 with quarterly bonus. Benefits and Perks
