Overview
The IRC is seeking an Intensive Case Management (ICM) Caseworker to address barriers to safety and wellness for particularly vulnerable refugees, asylees, and other ORR-eligible populations. The ICM caseworker provides strengths-based, client-centered case management to diverse refugee households and individuals, working with clients, colleagues, and external partners to develop service plans that meet individual needs and program requirements across a broad range of services. Location: San Jose, CA. USA Major Responsibilities
Determine client eligibility for ICM programming and conduct intake assessments, review program expectations, outcomes, and confidentiality with each potential client. Develop individualized, SMART self-sufficiency plans for each client enrolled in the ICM program. Evaluate client progress every six months during active program enrollment to determine progress toward goals and identify new needs and actions. Assist clients in navigating healthcare and social service systems, including helping clients apply for benefits and attend and schedule appointments. Conduct community outreach and maintain a strong network of service providers; maintain relationships with local and state refugee coordinators and health coordinators to ensure client needs are met. Provide clients with extended orientation on topics such as women’s health, transportation navigation, and local healthcare system navigation. Respond to client crises with guidance from a supervisor as needed. Assist clients with requesting financial assistance when applicable and track received financial assistance within case files. Maintain up-to-date documentation in client case files and in IRC’s database, Efforts to Outcomes (ETO). Attend relevant trainings and meetings at the local and national level. May train and lead the activities of volunteers and interns. Other duties as assigned. Job Requirements
Education : Bachelor’s degree in social work, public health, or related field of study preferred. Work Experience : Minimum of 1-2 years of case management experience with similar populations and/or client needs required. Demonstrated Skills & Competencies
Strong relationship building, diplomacy, and networking skills; ability to effectively build internal and external relationships. Demonstrated success working and communicating effectively in a multi-cultural environment. Self-starter with excellent problem-solving skills and ability to prioritize and manage time effectively. Attention to detail and accuracy in work product. Fluent in English required, both spoken and written; proficiency in client group languages preferred, including Dari/Pashto. Proficient in Microsoft Office applications and digital tools for data entry, research, and problem-solving. Valid driver’s license and access to a personal, insured vehicle. Working Environment
Combination of standard office environment, remote work, and field time within the service delivery area. May require occasional weekend and/or evening work. Compensation
Pay range: $29.10 - $30.00 per hour. Posted pay ranges apply to US-based candidates. Ranges are based on factors including labor market, job type, internal equity, and budget. Offers are calibrated by work location, candidate experience, and skills relative to requirements. Additional Information
