Overview
St. Paul's PACE Akaloa – Chula Vista, 630 L Street, Chula Vista, California, United States of America. This is an early access program for individuals 55 years or older who are living with chronic illness or disabilities and need coordinated medical care to continue living independently in their home and community. The healthcare teams at St. Paul’s PACE provide a hands-on approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone.
Job Description We are looking for Full-Time Physician Assistants or Nurse Practitioners! This is typically a Monday - Friday position in Chula Vista.
Job Summary: Under the clinical direction and supervision of the Medical Director provides primary care to participants. Performs physical assessments of participants and develops and contributes to implementing appropriate plans of care to St. Paul's PACE program participants. Evaluates participant’s physical complaints and provides appropriate treatment. Provides participant and caregiver teaching and education. In addition to the Primary Care Physician, participates as a member of the Interdisciplinary Team. Demonstrates the knowledge and skills necessary to assess, plan, care for, and provide services to frail elder participants according to assigned responsibilities and PACE standards.
Qualifications and Requirements
Education : Graduate of a school of professional physician assistant or nurse practitioner required. Licensed as physician assistant or nurse practitioner in the State of California. Certified as a physician assistant practitioner or nurse practitioner by a national certifying organization recognized by the State of California.
Experience : A minimum of one year's experience working with the frail and elderly population required. Two years experience as a physician assistant or nurse practitioner in a geriatric setting desirable. Shall have either training or related experience in the job assigned.
Possession of a valid California License as a Physician Assistant or Nurse Practitioner
Degree from an NCCPA certified program as a Physician Assistant or Nurse Practitioner program

Thorough knowledge of current concepts, theories, and practices related to home and community-based care for the elderly and disabled adults.
Working knowledge of the PACE regulations desired
Thorough knowledge of physical, mental and social needs of frail older adults.
Ability to lead and work with the interdisciplinary setting
Able to manage changing priorities per needs of the PACE program and the agency
At least one year of experience working with a frail or elderly population
