Overview Position: Full Time APP - Geriatrics
Department: Pace Clinic - Geriatric Medicine
Facility: Pace / Michigan
Location: Flint, Michigan
Schedule: Monday–Friday, 8:00 AM – 4:30 PM
Responsibilities
In collaboration with a physician, APPs provide expansion of individualized primary healthcare services by diagnosing and treating certain acute or chronic health conditions.
Obtain patient history and perform assessment via observation, interview and examination.
Order, perform and interpret diagnostic studies.
Perform preventative health assessments, screenings, immunizations and patient care.
Provide direct treatment and management of health conditions via referral to other healthcare providers or community resources.
Counsel and educate patients and their families/caregivers concerning preventative health, treatment options and community resources.
Document delivery of health care and nursing processes in accordance with specific specialty area standards and network nursing policy.
Job Duties
APP works mostly independently but under the direction of a supervising physician.
Call will be 1:6 but currently is 1:4 (Week on at a time).
Will provide services at Ascension Living Flint, MI clinic, and could include rounding on inpatients from time to time but not very often.
Typically see 4–6 patients in a clinic setting (2–3 routine assessments, plus 2 urgent visits daily).
Geriatric experience is a plus; requires candidates with at least 3+ years of primary care clinic experience.
Seeking candidates with strong initiative, critical thinking skills, emotional maturity and ability to work well within a team setting.
Willingness to learn new medical knowledge and skills when needed.
Experience in a geriatric setting, palliative care or hospice setting is a plus.
About PACE
The Program for All Inclusive Care (PACE) coordinates and provides preventive, primary, acute, and long-term care services so older individuals can continue living in the community.
Aged 55 years old or older and live in a PACE provider’s service area.
Certified by the state as meeting the need for nursing home level of care and able to live safely in the community with PACE services at enrollment.
PACE programs are covered by Medicare and Medicaid for eligible individuals; private pay options are also available.
Requirements
BLS Provider certification required prior to hire/date of transfer.
Nurse Practitioner certification required prior to hire/date of transfer.
Registered Nurse license required prior to hire/date of transfer.
Master's degree of Nursing required.
