Job Summary
Infusion clinical staff pharmacist - IV certification required. Central pharmacy operation, technician supervision, IV admixture of both hazardous and non-hazardous medication oversight. Responsible for covering the weekend and the operational needs of all infusion pharmacies. Oncology or infusion pharmacy experience preferred.

Experience and Education
Education: Bachelor of Science (BS) degree or Doctor of Pharmacy (PharmD) degree Experience: 1 year of pharmacy experience (English speaking, reading, writing) Licenses and Certifications: Eligible for licensure by Texas State Board of Pharmacy (TSBP) upon hire and Texas license with TSBP within 30 days; ACPE-IV Sterile Compounding certification; Basic Life Support (BLS) and, for inpatient positions, Advanced Cardiovascular Life Support (ACLS) within 180 days
Preferred
Experience 1 year of pharmacist experience
Job Duties
Perform duties in accordance with organizational and departmental policies, procedures, performance improvement objectives and environmental/infection control standards, and state and federal laws and regulations. Enter orders accurately, review/update medication profiles, and be cognizant of patient charges. Review medication orders for therapeutic appropriateness, interactions, and safety; seek clarification as needed and document interventions (e.g., ADR). Work independently, multi-task, problem-solve, and provide direction when needed; cross-train and maintain competency in multiple areas. Enforce formulary policy and participate in cost-effective use and selection of pharmaceuticals. Communicate drug information to physicians, nurses, etc.; assist in patient/family education and counseling; document clinical activities clearly for follow-up; contribute to patient satisfaction metrics. Manage technicians and other staff; validate accuracy of medications for distribution; audit controlled substances per law and policy; investigate discrepancies. Engage in professional development, participate in orientation and training, and take on additional projects as needed. Participate in the clinical program and support P&T initiatives; monitor target drugs for protocol changes; collect data and identify opportunities for improvement; perform therapeutic monitoring and dosage adjustments as needed. Notes: Duties may involve direct patient care, handling human-subjects research records, and/or maintaining patient or research records as applicable. Performs other duties as assigned.
