Clinical Staff Pharmacist, Acute Care, Variable Shift, Part-time — UT Southwestern Medical Center, Dallas, TX Responsibilities
Performs job responsibilities in accordance with organizational and departmental policies, procedures, performance improvement objectives and environmental/infection control standards as well as state and federal laws and regulations. Performs all computer order entry in an accurate, thorough and timely manner. Reviews/updates medication profiles (therapies, allergies, height, weight, etc.) and is cognizant of patient charges. Reviews medication orders for therapeutic appropriateness, duplications, interactions, overall effectiveness and safety. Seeks clarification if necessary and documents interventions (e.g., ADR). Works multi-tasking and problem solves independently with minimal supervision. Takes charge when necessary and can be relied upon for direction. Maintains competency in multiple areas through cross-training. Enforces the formulary system and follows policy when acquisition of non-formulary product is needed. Participates in cost-effective use and selection of pharmaceuticals. Communicates drug information to physicians, nurses, etc. in a courteous and professional manner. Assists in patient/family education and counseling regarding drug therapy and side effects. Documents clinical activities and summarizes cost savings and key information for follow-up by colleagues. Aims to improve patient satisfaction measured by appropriate metrics. Manages technicians and other supportive personnel. Delegates tasks and validates accuracy of medications from IV lab, central pharmacy, or decentralised dispensing machines. Audits controlled substances per policy and investigates discrepancies. Participates in clinical program growth, supports P&T initiatives, monitors protocols, and collects data for improvement opportunities. Performs therapeutic monitoring and recommends dosage adjustments based on renal function and pharmacokinetics. Other duties as assigned. May include direct patient interaction, research participation, and handling patient records as applicable. Qualifications
Education: Bachelor of Science (BS) or Doctor of Pharmacy (PharmD). Experience: 1 year of pharmacy experience; English proficiency (speaking, reading, writing). Licenses and Certifications: Eligible for licensure by Texas State Board of Pharmacy (TSBP) upon hire; Texas license with TSBP within 30 days; ACPE-IV Sterile Compounding certification; BLS certification; Advanced Cardiac Life Support (ACLS) as required for inpatient positions within 180 days. Preferred
Experience 1 year of pharmacy experience (note: included here for emphasis if applicable). Knowledge, Skills And Abilities
Basic computer skills and data entry; English proficiency; good general health and stress coping ability. Moderate physical effort: prolonged standing/walking, occasional lifting up to 25 lbs, reaching, bending, crouching. OSHA Category III. Physical Demands/Working Conditions
