Infusion investigational drug clinical staff pharmacist - IV certification required. Clinical Trial Unit infusion support, technician supervision, IV admixture of both hazardous and non-hazardous medication oversight. Responsible for covering the weekend and the operational needs of all infusion pharmacies. Oncology or infusion pharmacy experience preferred. Benefits

Education: Bachelor of Science (BS) degree or Doctor of Pharmacy (PharmD) degree Experience: 1 year of pharmacy experience Licenses and Certifications: Eligible for licensure by Texas State Board of Pharmacy (TSBP) upon hire and Texas license with TSBP within 30 days; ACPE-IV Sterile Compounding (IV) certification; Basic Life Support (BLS); Advanced Cardiac Life Support (ACLS) may be required for inpatient positions within 180 days Preferred

Performs job responsibilities in accordance with organizational and departmental policies, procedures, performance improvement objectives and environmental/infection control standards as well as state and federal laws and regulations. Performs all computer order entry in an accurate, thorough and timely manner. Reviews/updates medication profiles (therapies, allergies, height, weight, etc.) and is cognizant of patient charges. Reviews medication orders for therapeutic appropriateness, duplications, interactions, overall effectiveness and safety. Seeks clarification if necessary and documents interventions (i.e., ADR). Multitasks and problem solves independently and effectively with minimal supervision. Cross trained and multifunctional to maintain competency in multiple areas. Enforces the formulary system and follows departmental policy and procedure for non-formulary acquisitions. Participates in cost-effective use and selection of pharmaceuticals. Communicates drug information to physicians, nurses, etc., in a courteous and professional manner. Assists in patient/family education and counseling regarding drug therapy and side effects. Documents clinical activities and cost savings clearly for follow-up by fellow pharmacists. Manages technicians and other support personnel. Delegates tasks and validates medication accuracy for distribution from the IV lab, central pharmacy or automated dispensing machines. Audits and signs out all controlled substances in accordance with state/federal guidelines and policies. Participates in clinical program and supports P&T initiatives by monitoring target drugs for protocol discontinuation, substitution, IV to PO, etc. Collects data/measures and communicates opportunities for improvement. Performs therapeutic monitoring and dosage adjustments as needed based on renal function and pharmacokinetic calculations. Performs other duties as assigned. Security and EEO
