Overview
Clinical Pharmacist Specialist BMT – Medical City Dallas (HCA Healthcare). Full-time day shift, rotating weekends. BMT experience required. Qualified new candidates may be eligible for a sign-on bonus up to $30,000. Responsibilities
Interprets orders, transcribes or verifies them accurately and efficiently into the pharmacy computer system. Accurately compounds and dispenses pharmaceuticals. Monitors drug therapy regimens for contraindications, drug‑drug interactions, drug‑food interactions, allergies and appropriateness of drug and dose. Provides clinical consultation and clarification to practitioners; suggests appropriate, cost‑effective therapeutic alternatives as needed. Sustains the hospital formulary, minimizes nonformulary procurements, utilizes therapeutic interchanges and promotes rational drug therapy selection. Attends and actively participates in daily multidisciplinary and bedside rounds. Completes drug utilization reviews to promote rational drug therapy and implement improvements identified. Provides accurate and timely medication information to healthcare staff and patients. Leads and develops pharmacy clinical services such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing and other programs; develops, manages and mentors antimicrobial management program (AMP). Documents all clinical activities and interventions accurately and completely; mentors and educates and provides ongoing analysis of interventions for accurate documentation and reports. Supports and participates in quality improvement, risk management and patient safety initiatives and regulatory goals. Participates in design, collection and reporting of adverse drug reactions, drug utilization reviews, medication errors and pharmacy quality initiatives. Leads training and mentorship of pharmacists, students, residents, technicians, nurses and physicians; develops staff competency and addresses deficiencies. Maintains professional competency to meet current and future facility needs; provides expertise in a specialized area of pharmacy practice. Provides technical support for automated dispensing machines and pharmacy computer systems; supervises pharmacy operation activities. Works within the facility pharmacy schedule to interact with medical staff in patient care areas; intervenes in conflicts between pharmacy and medical staff as needed. Participates in disaster drills and Code Teams; maintains current BLS-ACLS or PALS as appropriate. Maintains controlled substance records and supports diversion prevention and monitoring; participates in reviews and audits. Ensures compliance with regulatory standards and participates in quality and regulatory reviews; leads process improvements. Supports achievement of pharmacy goals, participates in committees, and assists with staff performance evaluations. Assists with physician order sets, evidence-based guidelines, and pharmacy communications; supports pharmacy newsletters and drug information resources. Performs drug class reviews and drug monographs; participates in all aspects of formulary management and policy development. Participates in development and monitoring of operations initiatives and pharmacy policies and procedures. Qualifications
Education: B.S. in Pharmacy from an ACPE-accredited school OR a Doctor of Pharmacy from an ACPE-accredited college of pharmacy. Licenses/Certifications: Must have one of the following prior to start: current Texas Pharmacist license or eligible with Authorization to Test; ACLS/PALS/NALS as required for the role. Experience: Minimum 3 years as an Advanced Clinical Pharmacist or 5 years general hospital pharmacy experience; PGY2 residency or fellowship preferred; BCPS or other BPS certification.
