We are seeking a Clinical Pharmacist Specialist BMT PRN for our team to ensure that we continue to provide all patients with high quality, efficient care. We are an amazing team that supports each other and are seeking a phenomenal addition who values patient care. Responsibilities

This is a PRN position The Pharmacy Clinical/Operations Specialist BMT is responsible for participating and leading pharmacy clinical or operational programs, monitoring patient medication regimens, and providing appropriate interventions. The Clinical Specialist shall also assist in mentoring pharmacy staff and precepting students/residents and assist in the evaluation and implementation of clinical/operational programs. What you will do in this role: Interprets orders and transcribes or verifies them accurately into the pharmacy computer system Accurately compound and dispense pharmaceuticals Complies with facility policy and procedures and regulatory requirements Monitors drug therapy regimens for contraindications, interactions, allergies and appropriateness of drug and dose Provides clinical consultation and clarification to practitioners; suggests appropriate, cost-effective therapeutic alternatives Sustains the hospital formulary, minimizes nonformulary procurements, utilizes therapeutic interchanges Attends and actively participates in daily multidisciplinary and bedside rounds Completes drug utilization reviews to promote rational drug therapy; implements improvements Provides accurate and timely medication information to healthcare staff and patients Leads and develops pharmacy clinical services and programs as defined by Pharmacy Leadership; develop and mentor antimicrobial management program (AMP) Documents all clinical activities and interventions; mentoring and education; provides summary reports Supports quality improvement, risk management and patient safety initiatives Participates in adverse drug reactions, drug utilization reviews, medication errors and pharmacy quality initiatives Leads training and mentorship of pharmacists, staff, students; maintains staff competency and improvement plans Maintains professional competency; provides expertise in a specialized area of pharmacy practice Provides technical support for automated dispensing machines and pharmacy computer systems Supervises pharmacy operations and supports the pharmacy team Works in patient care areas; interacts with medical staff and nursing Intervenes in conflicts between pharmacy and medical staff regarding drug therapy Trains and participates with Disaster Team drills and real-event response Trains and participates with Code Teams; obtain/maintain BLS-ACLS or PALS as appropriate Maintains controlled substance records; participates in reviews and audits Ensures compliance with regulatory standards; participates in quality and regulatory reviews Prioritizes work; leads others to comply with medication turnaround time policy; identifies efficiency improvements Supervises and assists pharmacy technicians and support staff Participates in establishing pharmacy goals; collaborates with nursing and medical staff committees Assists with staff performance evaluations and may act as manager in charge as assigned Provides guidance on physician order sets and evidence-based guidelines Assists with pharmacy newsletter and written/electronic communications Performs drug class reviews and drug monographs; participates in formulary management Assists with development and monitoring of pharmacy operations initiatives Participates in policy/procedure development and review Qualifications

Education B.S. in Pharmacy from an ACPE-accredited School of Pharmacy OR a Doctor of Pharmacy from an ACPE-accredited college of pharmacy Licenses/Certifications Must have one of the following prior to the first day of employment: Current registered Pharmacist licensed by the TX State Board of Pharmacy; Licensed pharmacist in another state with proof of Authorization to Test (ATT) from the Texas State Board of Pharmacy; ACLS/PALS and NALS as required for critical care/ED depending on population served Experience Minimum of 3 years experience as an Advanced Clinical Pharmacist (in a specific area of focus) or at least 5 years of general hospital pharmacy experience; preferably within BMT OR completion of a PGY2 residency or fellowship. BCPS certification or other Board of Pharmacy Specialty (BPS) certification. Ongoing training and competencies as defined by service line, facility and department/unit.
