Care Manager / Designated Care Manager

at Sunrise Senior Living is responsible for providing the highest quality care and services to a consistent group of residents and their families in our assisted living/long term care and reminiscence neighborhoods. The Care Manager provides hands-on care and physical and emotional support as outlined in each resident's Individualized Service Plan (ISP) while maintaining a safe, home-like environment. This role demonstrates the Mission of Sunrise Senior Living, to champion quality of life for all seniors, in accordance with applicable laws, regulations, and Sunrise standards, policies, and procedures.

Job Description

Responsibilities & Qualifications

Essential Duties

As part of the Sunrise team, supporting our Mission, Principles of Service and Core Values is fundamental. The role includes, but is not limited to, the following essential functions:

Resident Care

Review, read, notate, and initial Daily Log to document and learn about pertinent resident information.

Participate in the development of ISPs and monthly updates.

Review designated assignments.

Be responsible for a designated group of residents during the shift, know where residents are, and physically check on them throughout the shift.

Observe, report, and document symptoms and conditions of residents for changes such as skin, behavior, alertness, weight, diet, and participation in activities.

Attend daily Cross Over meetings with the lead care manager.

Notify supervisor and resident care director if a resident has increased care needs.

Inform supervisor of any resident changes in condition.

Respond to security system and resident call bells promptly and take appropriate action including resetting call bells.

Greet guests, family members, residents, and team members; answer, direct and/or respond to phone calls professionally.

Assist with continence management and dispose of continence products in a sanitary manner.

Communicate with families and serve as a resource as needed.

Help residents maintain independence and physical safety and promote dignity in accordance with Resident Rights and Sunrise Principles of Service.

Participate in/lead and assist residents with activities of daily living (ADLs) and invite, encourage, and assist (IEA) residents to participate in activities as indicated on the ISP.

Engage residents in life skills and life enrichment activities, including reminiscence, throughout the day.

Respond with empathy, considering each resident’s communication patterns or history and basic needs.

Maintain an atmosphere of warmth, personal interest, and a calm environment; follow established safety regulations and standard care precautions.

Host and engage in daily activities with residents.

Risk Management and General Safety

Partner with the community team to ensure compliance with regulations related to occupational health and safety, promoting risk management programs and policies.

Follow safety procedures, including use of PPE, fire safety, SDS, and lockout/tagout as applicable.

Report all accidents/incidents and unsafe conditions/equipment immediately.

Keep cords, carts, equipment, and other hazards out of walkways and in compliance with fire codes.

Understand and practice proper handling and disposal of potentially infectious materials and exposure to hazardous substances.

Ensure oxygen tanks are stored safely and maintained at prescribed levels.

Housekeeping and Laundry Services

Maintain and clean resident rooms, including sanitation tasks and laundry as ISP dictates.

Maintain dining room linens and general common areas in a clean and tidy manner.

Dining Service

Serve meals in the dining room and assist with dining room setup, service, and cleanup as assigned.

Promote a pleasant dining experience and follow residents’ diets as indicated on the ISP/Confidential Diet Board.

Observe, document changes in dining habits, and respond to dining needs while upholding Sunrise hospitality standards.

Provide room service as needed and practice safe food handling at all times.

Specific Responsibilities for the Reminiscence Neighborhood Care Manager

IEA residents in life skills and enrichment activities per ISP and demographic profile; encourage afternoon social participation.

Incorporate"Creating Pleasant Days" into daily routines and blend residents’ life skills into activities.

Incorporate multi-sensory experiences into daily routines and participate in monthly letters with residents and families.

Training and Team Contribution

Work as part of a team; contribute to team goals and demonstrate the Team Member Credo.

Support residents through Sunrise Principles of Service and participate in engagement programs and surveys.

Attend regular meetings and trainings; comply with infection control and all required training for Sunrise standards.

Perform other duties as assigned.

Core Competencies

Ability to stay calm in difficult situations and manage multiple priorities.

Effective written and verbal communication; strong organizational and time-management skills.

Good judgment, problem solving and decision-making skills.

Experience and Qualifications

High School diploma/GED (as required by state/provincial regulations).

CPR and First Aid as required by regulations.

Must be at least 18 years of age.

Previous experience with seniors preferred; a desire to serve and care for seniors.

Ability to act in the resident’s best interest; drivers may need to sign the Driver job description for safety compliance.

Pre-employment Requirements
