Overview

Clinical Pharmacist Ambulatory role at Brown University Health. Reports to the Director Brown University Health Pharmacy, Director Specialty Pharmacy, or Ambulatory Manager as applicable. Under the supervision of the Pharmacist-In-Charge, Pharmacy Supervisor, Senior Clinical Pharmacist Specialist, or Clinical Coordinator, responsible for interpretation, evaluation, preparation, and dispensing of medications pursuant to a legitimate order or prescription, in keeping with established policies, procedures, standards of practice, and scope of ambulatory pharmacy services. Collaborates with multidisciplinary care providers to monitor, assess, and intervene as needed to ensure safe, appropriate, and cost-effective medication use. Oversees and reviews the work of pharmacy technicians, pharmacy interns, and ancillary support personnel. Responsibilities

Apply pharmacotherapeutic and pharmacokinetic principles to ensure safe and effective use of medications for the patient populations served. Ensure pharmacy services comply with policies, procedures and federal/state regulations. Receive patient medication orders or prescriptions from practitioners in the outpatient setting and review for therapeutic appropriateness in relation to clinical status, concomitant therapy, diet, and developmental stage. Contact the prescriber to initiate interventions to maximize therapeutic efficacy, minimize risks, and optimize cost effectiveness. Provide education on medication use to patients and families; share information about drugs with physicians, nurses, and other healthcare practitioners. Participate in education and training of healthcare practitioners, pharmacists, interns, technicians, students, and other care providers; may serve as a preceptor to residents and interns. Participate in continuous quality assessment and improvement activities and total quality management initiatives, including adverse drug reaction reporting and medication incident reporting. Maintain professional competence through staff development, ongoing study, and professional affiliations. Perform immunizations or medication administrations as allowed by Rhode Island Board of Pharmacy Rules and Regulations, dependent on position and responsibilities. Perform other duties as assigned. Specific Retail and Specialty Pharmacy responsibilities: review and enter orders in the pharmacy information system; verify patient parameters; dispense medications; review formulations for compounding; ensure proper documentation and record-keeping. Specific Specialty Pharmacy responsibilities: adhere to SDPMP policies, perform patient outreach and assessments, participate in developing and executing patient Plans of Care. MAP responsibilities: provide MAP refill authorization services, navigate EHR for MAP questions, document investigations and authorizations, provide alternate therapy recommendations as needed, monitor MAP technicians, identify operational trends, and ensure quality care standards. Clinic-based program responsibilities: monitor lab results, interpret findings, and adjust regimens per protocol and agreements; identify out-of-scope issues and triage appropriately.

Minimum Qualifications

Bachelor of Science in Pharmacy or Doctor of Pharmacy from an accredited college of pharmacy. Current, unrestricted pharmacist licensure in Rhode Island (or 90-day temporary license with full licensure obtained before expiration). Immunization Certification (ACPE approved) and American Heart Association BLS certification within 6 months of start date, if position involves immunizations/medication administrations. Two to three years of progressively responsible related experience preferred; knowledge of pharmacy information systems, EHRs, automated dispensing technology, and PCs. Ability to travel between facilities/site as required.

Work Environment and Additional Details

Extended periods of standing and walking; manual dexterity for computer operation. Independent action within department policies with escalation for complex issues. No supervisory responsibilities for this role.
